A mature leader starts discussing with people who are involved in the process and listens carefully. By listening he immediately started thinking about what is wrong or what may go wrong. Then star asking the question and go deeper. If he found the cause of failure the immediately he started evaluation to finding the gap between practical vs procedure. Immediately start discussing with people about the logical and ethical CAPA to avoid the same in future.

Monitoring needs every day, every shift needs to talk with people as much as possible and also tell them the importance of CAPA until it becomes practice.
