Recreating accident scenario helps in determining what,where,how,when,why got missed and accordingly actions need to be paced up.GEMBA analysis needs to be recorded in a meeting with a brainstorming session focusing on aspect, impact of hazard.RACI method needs to be used for corrective actions and follow up in order make actions systematic and must be standardized.A review mechanism at all levels must be made to ensure corrective actions are followed and are adequate.

One mistake I see is when a company only makes changes if someone was injured. Instead, if a near-miss occurs, what you should do is consider how severe someone could have been hurt. From my experience, the most important aspect would be identifying the behaviours which lead to the accident . Unless we understand that , it is more common that we might miss addressing the root cause and land up with potential opportunity for recurrence . Uma cultura de segurança começa na liderança. Os líderes têm que entender que eles são os responsáveis pelo tema e não a área de segurança do trabalho. A área ajuda nas análises técnicas mas os líderes são o dono do processo. Se um acidente acontece numa linha, o supervisor tem que ser responsabilizado , ou por não ter treinado o colaborador, ou por não dar exemplo de atitude segura ou simplesmente por não focar no tema no dia a dia.
