Where to start? If it's a facility follow the 'reporting a problem' path laid out in their patient documents. Recognize few MDs are actually employed by the facility but have 'priveledges' to perform medicine there; however, the facility is a good place to start. They don't want to be known for a bad actors outcomes. If they themselves do not take the next steps they can tell who to contact. Other resources are the State Medical/Nursing/Pharmacy/other licensing board Board. It is a moral and ethical duty to report harm.
