In my opinion by using sing QHSE indicators we can easily to identify, prioritize improvement opportunities. for this you need to define KPIs like product defects, claims, audits variation incident, absenteeism hazards, risks emissions, waste, energy consumptions etc clearly, once you defined these objectives you need to track and start collecting data and records with surveys, reports, audits etc, for analysis for trends and patterns on regular basis leads to improvement for management programs for improvements for QHSE. in addition you need to consider respective legal and other applicable requirements and compliances. continually monitoring reviews and actions helps to find and prioritize opportunities for improvement.
