1) Code quality is not a one day job, it is a continous process. It should be started even before project starts by hiring competent candidates who understands what they are doing. 2)From day 1, understanding the work, the user story, epic , feature should be an integrated part of the daily work. 3) Test Cases should be reviewed by the writer first, then Sr QA. If needed functional test cases should be sent to review of Product Owner. 4) If the above process is followed religiously then code quality question will not arise.

Understand the requirements well - Understanding the requirements very early in the game is the key to ensure you are on top of delivery with quality. Have enough dialog with cross functional stakeholders to elicit the requirement. Also ensure there is a traceability from requirements to design to code to test cases
