While it may be one of the most difficult exercises, it sets the stage for a more objective analysis. Reflection will help you understand your own role in the outcome and what could have been done differently. It takes a lot of vulnerability to do face this, learn from it and grow but it is worth it as you avoid similar mistakes and lead from a place of growth in future situations.

" Who made the mistake?" - Absolutely wrong question to ask during failure understanding. " What went wrong & why" is the correct one. Also understand, What went right & why? Compare expected outcome & actual wrt degree of deviation. Is it complete failure or 80-90 % achievement? 1-2 teams amongst all stakeholders failed or cumulative? What went right & why? Face the mirror & it will narrate the story!

Data does not speak itself unless you understand & convert it into information. Info is then impersonal, deeper you look, clearer will be the picture. Ask it same question in different manner & amazing patterns / pictures will emerge. Gantt charts used for project tracking, periodic updates, vendor reports, red flags, unfilled reports, overlooked cautionary notes, deviations etc., all will speak up.

Data when converted into information will help you understand what you might have missed or overlooked before failure. It can serve you only as guidance for future but will not help reverse the failure. Does data analysis help you with patterns of teams, individuals, connectivity, interdependence, ripple effect, causes, trigger points etc.? Yes, very likely, only if discipline has been maintained to generating data from the start of the project. Analysis will pinpoint what went wrong, when n where? Sometimes by Whom as well. Lessons learnt must be shared across the organization to avoid recurrence & also for implementing Best Practices implemented during the project. Learn from failures!

As the title of this step suggests, always consider the "five-foot rule" when addressing failures. Engage those closest to the problem, i.e., the people within five feet of the problem—they have the most accurate insights and valuable input. They do the job daily and have the best view of what is occurring (or going wrong). While you may have experience in that role from years ago, trust that the job has evolved, and those doing it today know it best. "Meaning?" sends you deeper - understanding what they mean to ensure you're speaking the same language. "such as?" sends yo wider by encouraging them to give you examples. Most of missed communication is become someone made an assumption based on how they understood it rather than bothering to dig deeper.

When does a leader fail? It is a complex question and I would say that it depends on those involved, the situation, and the objectives set. The Royal Academy dictionary says that it is "having an adverse result in a business." That is, the definition is results-oriented. But adverse results could be in the process of reaching the goal: he was unable to form high-performance teams, there is no fluid communication, he was eventually unable to motivate the team or he did not know how to delegate management tasks in a timely manner. If leaders become leaders, it is because there was a path and it is very hard to speak of "failure." Given the circumstances, the prepared and recognized leader may have "setbacks" or eventually, a failed decision.

I have found journaling to be the best way to monitor progress. Identify your intention for the day. Write down your top 3 goals for the day. Write down what you are uncomfortable about and determine how to address this item. Write down where you won each day and and what you are most grateful for each day. If. you are serious about journaling, you will see a difference in your ability to lead and monitor your progress.
