People who are part of Agile Team should be crystal clear what is expected out of them and Product Owner should help and make requirements as much clearly defined as possible. Then there are metrics which measure a teams performance, however we should not be adamant to measure output quantitatively.

Once what is defined, how is the team's responsibility as Agile Team is self managing, self sufficient and might not need external interferences and support.

There should not be any bias within the team. The TEAM should be responsible for success or failure, no blame game and no escape goating. May be we decided that we will deliver 20 points in the sprint but due to other priority tasks it may increase and something else can be prioritize from back log and something from sprint can be removed. Team should be ready for such changes.

Similarly, Agile Team should also be open for adaptability, as there is no fix them all formula. Every situation is different and the solution will also be different, it might be that the core problem remains common. The Adaptability Quotient (AQ) can be very well utilised to gauge team's adaptability. It is not a mainstream metric, however it helps us to understand our team better.
