Clinical trials revolve around an approvable endpoint. Many think that asking agency for guidance surrounding the endpoint is wise, but the truth is it could open up pandoras box. The key is to not overstate the claim on your label. Using cancer as an example maybe you don't want to use progression free survival because that takes forever. This is the industry standard but why not use circulating tumor cells (CTC's). The common mistake is not telling the agency what you want instead of letting them influence your decision. You know the trial better than they do. The key is to have a trial designed to your drug and its capabilities instead of one that caters to all your competitors. Be directive with the agency regarding endpoints & design.

In my recent clinical trial for a novel diabetes management drug, defining clear objectives and outcomes played a pivotal role. Many collaborators are needed to complete a clinical trial. One common mistake that many young drug developers make is completely trusting their collaborators to do what they were hired to do. There needs to be checks and balances. If you hire a CRO you need to hire someone whose sole job is to question everything the CRO does. These checks and balances will keep inefficiencies out of the clinical trial development. For example, a CRO may add an extra criteria in screening that may make sense for a better trial outcome but it may cost you in the ability to recruit patients thereby requiring more clinical trial sites. This is one example of why it is so important to have a company monitor attached to each of your collaborators.

For sponsors & CROs to ensure effective monitoring and the successful conduct of clinical trials, choosing experienced and competent medical monitors is crucial. During the selection process, elements including knowledge, communication abilities, attention to detail, regulatory compliance, and experience in clinical site monitoring should be considered. The overall performance of medical monitors is also influenced by clear monitoring strategies, ongoing training, timely data reviews, successful collaboration, and effective communication. CROs may improve participant safety, guarantee data integrity, and produce positive results in their studies by carefully considering these elements and putting effective monitoring systems into practice.

Once you have found participants that fit your trial's criteria, you need to keep them engaged. This will help improve their experience, reduce their participation burden and hopefully prevent early dropouts. Technology can be a huge help here to properly inform participants of their roles, risks and responsibilities in the trial right up front and gather and more "informed" consent. Next, provide options on how they can participate via on-site visits, telehealth, home or local health providers and mobile apps to stay on track. Automate scheduling and reminders for important tasks and provide a mobile app or patient portal to give the participant easy access to everything they need. Sponsors should get better data as a result!

In order to attract/engage & retain potential participants you need make sure the patient flow is as smooth, clear, succinct and engaging as possible. This starts right at the beginning with the marketing strategy - how are you going to find these individuals?. Each study is different of course, but the key is to know your market segmentation and adopt a strategy that will reach, and engage, this population quickly. A clear call to action is required and that participant then needs a smooth, easy to navigate and fully informed route to participate in the study. In short, the patient experience is crucial from start to finish. If this is done correctly you give yourself the best possible chance to deliver those enrolment targets.

All parties need to be very transparent with the time in takes for trials to be completed. Too often, service providers will reduce the timelines and budget to make their organisation more attractive to Sponsor companies, only to find their timelines run way over with poor quality work. This does nothing for the provider’s reputation, business integrity and most importantly, the patients that urgently need the treatment to market. Being honest about when you can deliver is also key to delivering trial ‘on time’. On the flip side, sponsor companies need to listen and rely on the experience of those that have walked the path to ensure that their trial does not get unnecessarily delayed due to factors that could have been avoided.
