Lack of communication amongst the teams, working in silos, dependency mis-alignment. 3. Poor and/or lack of thorough validation at design stage. 4. Neither Agile nor Waterfall nor Hybrid, chaotic development methodology. 5. Lack of team cohesiveness, misaligned to the project vision and goal. 6. Lack of resources/poor quality resources. 7. Long list of planning assumptions not validated on time.

Planning and Tracking - plays vital role starting from procuring the software, skilled labor till we have collaborate with all the downstream and upstream teams at program level. Falling behind schedule is generally a result of many factors but I find lack of alignment on the goals, unclear scope, and a lack of a resource plan to be the areas to address to avoid schedule delays. Agile teams don't have typical 'projects'. A better question might be - why do agile teams fail to 'deliver' approved functionality every 'iteration'? Understandably, there are many forces at work in the IT industry - that have chosen to 'propagate' the need for typical 'project management'. What seasoned coaches understand is - that the 'flow rate' of delivering functionality with high quality in shorter increments - has proven to be a better insurance policy against 'projects' falling behind schedule. In affect, the 'project' is the 'iteration' - a very short timeframe by comparison to a 'multi-week/month/year' project. This reduction in risk is at the heart of successful agile teams. Establishing a consistent 'delivery' cadence is key.

5 Unrealistic expectations

Unrealistic expectations can lead to frustration, dissatisfaction, or disappointment. Low morale can affect the productivity, performance, and quality of your team and your project. To boost morale, you should provide strong and supportive leadership, recognize and reward your team members, balance the workload and the resources, clarify the roles and responsibilities, and resolve any issues or conflicts quickly and respectfully.
