    Studies presented this week at #ACEP24 challenge the new dogma that SEP-1 fluid resuscitation guidelines have no basis in science. Ward & colleagues presented a systematic review evaluating the effect of 30mL/kg on patient outcomes. Their analysis showed that patients receiving less than 20mL/kg had increased risk of mortality, while patients receiving 30ml/kg within 2-3 hours have the lowest mortality. Similarly, Piehl (that’s me) and colleagues evaluated fluid timing for septic shock in a large single-center study at WakeMed and showed almost exactly the same results: for patients with septic shock the lowest mortality was seen when fluid was initiated in the first hour and patients got at least 30mL/kg by the 3rd hour. Our recent abstract at #SAEM 2024 showed the lowest odds of mortality among patients who received 20-30mL/kg in the first hour. These findings are consistent with a retrospective study and a meta-analysis by Ward & Kuttab. Studies like CLOVERS confuse the message about “liberal” vs “restrictive” since they have nothing to do with resuscitation in the early minutes of care, but instead enroll patients many hours after their initial presentation. While not everyone needs protocolized fluid administration, and we should all be treating shock early and monitoring our patients at the bedside to observe response to fluid (ideally guided by ultrasound!), these studies show 30mL/kg is likely safe and effective. https://lnkd.in/eyy2V4_V https://lnkd.in/euMnkNCN   https://lnkd.in/eYBJmmWX
