  • Jen was recently able to participate in Senior PharmAssist’s luncheon celebrating 30 years of service! During the reception, our dear client Sue was featured in a very sweet video that highlighted how this great community resource has helped her. Senior PharmAssist is available for Durham residents and helps to ensure older adults have access to necessary medications. They provide financial assistance, medication reviews and Medicare insurance counseling. Please take the time to share this amazing resource with your friends and neighbors! #durham #seniorpharmassist #seniorcare #aginginplace #healtheducation #MedicationManagement #trianglenc

  • Meet Tom. Tom is an artist living with dementia. Tom has been contending with recent brain changes that are affecting many aspects of his life and impacting his sense of self. As part of an occupational therapy visit, our amazing therapist brought him to the Nasher Museum to experience the Reflections Program - a conversational art tour with activities designed for those with dementia and their care partners. The JOY emanating from Tom speaks for itself. See the next slide for more information on the Reflections Program. #occupationaltherapy #SeniorCare #nashermuseum #dementia #art #dukeuniversity #durham

  • Our occupational therapist, Samantha, kicked off the Durham Parks and Rec Mature Adults “Lunch and Learn Series” with a presentation on fall prevention and home modifications. Since September is Fall Prevention Month, knowledge is key to staying safe in the community! We love supporting our local communities and helping residents with strategies to age in place successfully. If you or someone you know could benefit from a home safety assessment or in-home therapy services to reduce the risk of falls, reach out to Active at Home today! #durham #fallprevention #fallrisk #homesafety #aginginplace #occupationaltherapy

  • Meet Joan. Joan is working on higher level balance activities in physical therapy after sustaining a fall. In this picture, Joan is working on dual tasking - bouncing a ball as she walks forward. It’s harder than it looks for most older adults and often times ends in fits of laughter. Our Active At Home therapists love to come up with engaging and fun exercises to keep clients motivated while also working on strength, balance and endurance. We offer both physical and occupational therapy in a mobile outpatient model which means we come right to you! • It’s all about the simple things, right? During her visit, our wonderful companion Liz brought bubbles for Ms. Bev to blow around the courtyard of her apartment complex. It was so heartwarming to hear about all of the community interactions that resulted from them doing this. Ms. Bev had a smile on her face the whole time and significantly boosted her mood. This is why we love companion care. Don’t forget that Active At Home only has a 2-hour minimum per shift! Call us today to learn more! 🌻 #Companioncare #aginginplace #seniorcare #durham #chapelhill #hillsborough

  • As a physical therapist, I notice quite frequently that the equipment clients are using is less than ideal. Some of the more common problems are brakes on rollators are not locking into place, wheels are not turning easily and wheelchairs are not appropriately fitted to client's needs. This can be a safety concern, leading to falls or other issues. So, I decided to educate myself on how to help clients FIX the problem, as most of them do not want to buy a new piece of equipment. I started volunteering with the Health Equipment Loan Program (HELP) in Durham to learn how to repair equipment and to be involved in an outreach program parallel to my field of expertise. Here’s a picture of me completing my first unassisted repair!!! I have a wonderful mentor there and he’s starting to let me tackle projects independently. HELP is a loan equipment program for Durham County residents. Their space is stocked with rollators, transport chairs, bedside commodes, adaptive equipment for ADL’s, knee scooters and wheelchairs. HELP accepts equipment for donations, ensures equipment is fit for use, and then sterilizes the equipment before loaning it out to the community. HELP is open on Tuesdays from 10-2 and Fridays from 1-5 pm. Check them out if you know someone who could benefit from this amazing program! #physicaltherapy #activitiesofdailyliving #medicalequipment #occupationaltherapy #durham

  • Our flyer says it all! Active At Home performs PT and OT in the comfort of your own home and no therapy referral is required! We perform these services under the Medicare Part B, mobile outpatient benefit. With a supplemental plan, oftentimes both services are covered at little to no cost to the client. While we don’t need a therapy referral to initiate services, our team communicates the plan of care and any progress or concerns to your doctor. Clients enjoy the ability to tackle functional activities head on with real life practice and the mere convenience of reducing time/energy heading to another appointment. Do you know anyone that would benefit from this amazing service? Please have them call our office at 984-837-0991 to learn more! #physicaltherapy #occupationaltherapy #chapelhill #Durham #hillsborough #raleighnc #trianglenc #AgingInPlace #seniorcare

  • Provider Spotlight - Meet Kacey! Hello, my name is Kacey. I graduated from the University of New Hampshire in 2021 with my master's in occupational therapy. The pandemic years may have stolen my graduation ceremony, but they did not slow me down. While in school I worked in an assisted living/memory care where I formed meaningful relationships with the residents and my heart grew for people living with dementia and their loved ones. I also took on an additional fieldwork experience through the Primary Care Behavioral Health grant. This gave me the opportunity to work with the behavioral health team at Concord Hospital Family Health Center caring for people experiencing chronic pain and a variety of mental health concerns including depression, anxiety, hoarding, ADHD, and substance use. From NH I moved to NC to complete a Geriatric Mental Health fellowship at the Durham VA. Working with veterans was an honor and a privilege. I dug into occupational therapy's roots as a mental health profession (did you know OT got its start in mental health?) and found a real passion for helping people navigate not just the physical challenges that impact their function, but also those barriers that are a little harder to see. I gained experience in a variety of settings including the homeless program, inpatient acute, inpatient psychiatry, outpatient memory group, a dementia support program, home based primary care, and a primary care team. I am honored to be a new member of the Active at Home family and excited to meet you. My goal as an OT is to help people break down barriers and find satisfaction doing the things they find important to them whether that's enjoying a meal with family, re-engaging in a hobby like knitting, getting dressed to go to church, or maybe discovering something new to fill their time with meaning and purpose. #occupationaltherapy #aginginplace #durham #chapelhill #hillsborough #trianglenc #seniorcare

  • Our physical and occupational therapists recently participated in a Qi-Gong team building event led by Jen’s mother-in-law, Stannie! Stannie instructed our therapists in a series of standing postures that focus on coordinating movement with breath and intention. These exercises are great for postural awareness, gentle ROM through all the joints as well as balance. Our Active At Home team thoroughly enjoyed this time together and each therapist walked away with a client in mind that would benefit from this practice. #PhysicalTherapy #occupationaltherapy #qigong #aginginplace #durham #chapelhill #Hillsborough
