About us
Multimorbidity is a common condition in older age, and can substantially influence individuals’ health and quality of life, making management more difficult. A single-disease approach with fragmented care is still prevalent in current healthcare systems despite multimorbidity representing a heterogeneous spectrum of disease combination(s). On this background, the novel approach underpinning the AFFIRMO project is to focus on clusters of multimorbidity where atrial fibrillation(AF) represents one of the chronic conditions.

On July 2nd, 132 people participated in the joint webinar “Joining efforts to develop Geriatric Medicine across Europe: The experience of EU-funded projects”. 🌍This meeting focused on three European projects on aging - PROGRAMMING CA21122, I-CARE4OLD EU Project and AFFIRMO. 🔹 Marina Kotsani talked about rational, goals and challenges of PROGRAMMING CA21122; 🔸 Rosa Liperoti explained the idea of I CARE 4 OLD project (development of an innovative decision-support tool for health care professionals, to enable personalized care); 🔹 Soren Paske gave a lecture on AFFIRMO project (iABC platform); 🔸 Cristina Polidori talked about UNION consortium, the project focused on frailty, and UNION’S research and training objectives. 🎉 Exciting times at AFFIRMO! 🎉 On May 27 and 28, 2024, we held our third face-to-face Consortium meeting at the Heart Care Foundation (HCF) in the beautiful city of Florence, Italy! 🇮🇹 During these two productive days, we presented the significant progress made in the second year of our project and delved into future activities and plans for each of our work packages. It was an engaging and rewarding experience reconnecting with all the partners, sharing thought-provoking conversations, and visiting the HCF. Looking forward to present & exchange insights at the 27th Nordic Congress of Gerontology in Stockholm, Sweden, this week! The congress will be hosted by Karolinska Institutet (KI), internationally renowned for its research and one of our consortium partners. 📅 On Friday 16 June, Davide Liborio Vetrano (Karolinska Institutet) will chair a symposium entitled 'Risk stratification of multimorbid patients across the care continuum: insights from three EU-funded projects'. For this symposium, I-CARE4OLD EU Project will share its insights together with EU Horizon 2020 projects AFFIRMO (Mirko Petrovic; Lu Dai) and EHRA-PATHS. 📅On Wednesday 12 June, Johanna Edgren from the Finnish Institute for Health and Welfare (THL) will present a poster on 'Multimorbidity in home care clients with cognitive decline and its association with use of health and care services'. Co-authors of the poster are Jokke Häsä and Mari Aaltonen (both from THL). 💬 We hope to engage with many fellow researchers, but also practitioners, professionals, and policymakers at the event!
