I have been accepting Medicare in my practice since 2008 💅 Some of my most significant referral sources are local endocrinologists & PCPs. 💵 Plus, Sis, the rates for Medicare are quite awesome. 😎 Hear me out on this one ... The Affordable Care Act (ACA) has been a godsend to dietitians. 👏 When the ACA went into effect in 2015, it changed the game for dietitians in private practice. ⏰ When I first started accepting insurance in 2008 (straight out of my internship!) the reimbursement rates were lousy & just MNT for diseases like diabetes, renal disease & eating disorders were covered. Fast forward to 2023 ⏩ As a result of the ACA, preventative coverage for MNT is covered more than ever. 😍 Plus, when billing for preventative services, you don’t even need a referral. I don’t know about you, but getting a referral can be SUCH a pain in the ass. So as long as you code with preventative codes, there is no need to waste your precious little time 🥳 🤩 More importantly, MANY of my patients have unlimited visits with NO COPAY & NO DEDUCTIBLE. This is HUGE. What’s the deal with re-credentialing? 🧐 ➡️ Are you a dietitian who is currently working for someone else? ➡️ And did that someone else credential you? Are you scratching your head, wondering whether or not your credentialing status is transferable to your badass practice? The answer is - it depends 🤷♀️ Unfortunately, if someone else credentialed you, most insurance companies will require you to re-credential (or go through the contracting process again!) to bill on your own. ✅ Therefore, my suggestions are: 1️⃣ Update your CAQH application with your practice’s information, including your EIN of your practice 2️⃣ Call each insurance company by calling the provider line & tell them your intentions They will then best direct you to the next steps. You can be credentialed on your own and be credentialed with your current employer 👏 That is NO biggie at all. 😒 But more often than not, if you go on your own (which I am cheering you on to do!), you will need to start from scratch. 🙌 Hands up for RDs setting boundaries 🙌 Why is it that we RDs have such issues setting boundaries? I think it’s just that we care so dang much about our patients & always want to do the right thing - even at the expense of our sanity 😳 While this is definitely an admirable quality, it doesn’t always translate linearly to making $$$$$$. 😖 In fact - many RDs who fail to establish clear boundaries end up feeling like they have been hit by a Mack truck at the end of the day - beaten, ragged, busted, & broke. Are you feeling me here? I should know because when I first started my insurance-based practice, I was the ultimate sucker. I worked when it was convenient for my patients (Hello, 6:00 an appointment on a Saturday morning!), did a meal plan for ‘free’, & spent hours on end responding to patients' ‘urgent’ emails with novellas 🫠 While my patients ‘loved’ the services I was offering, I was overworked & not making my worth. 🧐 This made me question why I even went into Private Practice in the first place. I used to think I might as well work at Target - at least I have set hours & killer discounts! Fast-forward to the present day ⏭️ I now run a multi-six figure practice, only ‘working’ three days a week, with CRYSTAL clear policies and boundaries in place. 💪 You step to my boundaries and trust me - I will go all sorts of gangsta on you. 👏 Hands up for badass reimbursement coaches. If you guys have been following me for a while, you know I keep it REAL. So while I know dietitians are a super-intelligent bunch & are good at figuring shit out - the whole reimbursement process is downright overwhelming. ➡️ Misconception #123,245 about dietitians accepting insurance in your practice …. If you accept insurance in your practice, your patients will be less compliant because they aren’t paying out of pocket for your services and, therefore, have no ‘skin’ in the game. I die a slow death every time a dietitian presents this as an excuse for WHY they are not going to accept insurance in their practice 😫 Take something like the weight loss industry ... 💰 The weight industry is a billion-dollar industry. People throw money boatloads of $$$$$ at short-term fixes every day with minimal ‘success.’ So why do you think it’s any different if you offer self-pay MNT services? Look. Human behavior is complicated 🤯 Throw people’s relationship with food into the mix & this adds another layer of complexity. 🙃 While we like to believe people are motivated on some level by their financial commitment, this is far from the truth. Low-quality experiences with your patients don’t happen because you accept insurance. They happen because you fail to establish standards in your practice 😳 Your systems teach your patients how you want to be treated. ✅ In addition, these same systems define your and your patients' expectations. 🙌 Hands up for strong independent females accomplishing their dreams on their OWN terms 🙌 This is one of the main reasons I decided to start my private practice.
