  • Kalvin’s story is both inspiring and heartbreaking, highlighting the profound impact of a traumatic brain injury and the critical role of community support in recovery. At Aptiva Care Management, we are dedicated to guiding individuals and their families through these challenging times, ensuring they receive the care and support necessary for the best possible outcome. Kalvin’s journey began on February 15, 2023, when a devastating car accident left him with a severe traumatic brain injury at just 23 years old. Inadequate initial care led to his transfer to Craig Hospital in Denver, where a 60-day stay turned into a 10-month ordeal due to the complexity of his condition. After stabilization, Kalvin moved to Neuro Restorative in Orange County for comprehensive therapy and rehabilitation before returning home to his mother, Debbie, and stepdad, Ron. Significant home modifications were needed to accommodate his wheelchair use. Recognizing the financial strain on the family, Aptiva Care Management sought support through donations and partnered with The Side House Foundation. This collaboration provided essential home modifications, transportation, aqua therapy sessions, socialization outings, and covered medications not covered by insurance. Aptiva Care Management coordinated these efforts to enhance Kalvin’s quality of life and support his reunion with his loved ones. We launched a fundraising campaign on Instagram, offering T-shirts and wristbands with the message “Kalvin’s Mission Possible!” for a $50 donation. The generosity of our supporters has been instrumental in helping Kalvin return home and be with his family, which is crucial for his well-being and recovery.   • Chris Truchan and his family have been facing an incredibly challenging journey since his diagnosis with Progressive Juvenile-Onset Amyotrophic Lateral Sclerosis (ALS) in March 2021 at the age of 23. Juvenile ALS is a progressive and ultimately terminal neuromuscular condition that is diagnosed in less than 1 out of every 1 million people. Since Chris has had ALS for 3 years, he has lost the ability to move any part of his body and is now on a ventilator. Aptiva Care Management met Chris when he was in a hospital, which was certainly not the best environment for him. Chris needed to be around his loving family and friends in a supportive atmosphere for his well-being. Aptiva facilitated his transfer from the hospital back to his home. This involved extensive planning to ensure that all necessary medical equipment, supplies, and home modifications were in place to create a “hospital-at-home” setting. Aptiva also initiated a GoFundMe campaign to assist with the substantial financial burden associated with ALS, which is known for its high medical costs and the immense stress it places on families. Contributions to this campaign help cover various expenses such as medical testing, copays, medications, and at-home care. The Truchan family remains deeply grateful for any support provided.   • At Aptiva Care Management, we are dedicated to guiding individuals and families through challenging medical crises. During a recent visit to Neuro Restorative in Tustin, California, where we were treating another patient, we had the privilege of meeting Desiree Myers. Desiree, a passionate and talented pastry chef, who had big plans to make significant strides in the culinary world. Unfortunately, in April 2024, at just 35 years old, she experienced a major stroke affecting her lower-left brain stem. This sudden event required immediate emergency care and an extended hospital stay. Despite ongoing tests, the exact cause of the stroke remains unknown, and Desiree faces a host of challenges as she continues her long road to recovery. Desiree has been working diligently with physical therapists to regain function on her right side and has recently returned home to continue her therapy on an outpatient basis. Her ultimate goal is to regain her independence, return to work, and achieve a sense of normalcy in her life. While at Neuro Restorative, Aptiva Care Management, in partnership with The Side House Foundation, had been actively involved in supporting Desiree. We assisted her in navigating the complex medical system, completing forms for government assistance programs, and providing medical advice and expertise. We also maintained regular check-ins to ensure she received any additional support she might need. In addition to these efforts, we helped Desiree set up social media accounts and GoFundMe account, which has provided crucial financial support. However, she still requires additional donations to cover the costs of further tests to determine the cause of her stroke, ongoing medical care, and home modifications that are not covered by insurance.   • Aptiva Care Management, we specialize in supporting individuals and families navigating medical crises. During a routine visit to Neuro Restorative in Tustin, California to check on another patient, we had the opportunity to meet Jason Vititoe and his family. Jason’s story began in August 2022 when he received a life-changing diagnosis: an invasive brain tumor, confirmed through CT scans and MRIs. This devastating news brought with it the harsh reality of an incurable cancer, significantly impacting Jason’s life and that of his family. Despite the challenges, Jason’s goal was to return home to his loving wife and three young children. This dream came true this past Father’s Day, a heartwarming gift for Jason and his family. However, getting Jason home required significant modifications to their living space to accommodate his medical needs, including adjustments to the bedroom and bathroom, installation of a monitoring system, and acquisition of necessary medical equipment and supplies. Recognizing the financial burden on the family, Aptiva stepped in to offer support. We reached out to various donors and were fortunate to partner with The Side House Foundation. Thanks to their generosity, much of the needed equipment and modifications were provided. Aptiva also contributed through time, medical consultation and patient advocacy. To further assist the family, we launched a fundraising campaign on Instagram, featuring T-Shirts and wristbands with the message “Surging through the storm w/Jay!” available with a $50 donation. The support we have received has been crucial in helping Jason return home and be with his family, which is vital for his well-being.
