  • Police officers are the best patients. And firefighters. Our favorite are pilots. They have one thing in common: they never want to be in the doctor's office. When they do show up, it's only because they can't avoid it, and even then, all they want is to get back to work. Take one of our police officer patients, for example. He's a breacher—his job is to crash through doors so his team can secure the area. During one particularly intense breach, his right arm suddenly dropped, and he couldn't move it. He had a complete rupture of the supraspinatus muscle. After a long recovery, he was cleared for a trial of full duty. At his final appointment, he shared that per department policy, he had to see the "county doc" for his official clearance. His feedback? "The doctor didn’t even touch me! He did no exam, nothing." Which got us thinking—if employers are sending their employees to practitioners who can't be bothered to do an exam, wouldn’t telemedicine be a better option? The officer could do the visit in his patrol car during lunch. A teledoc might even have him do 20 push-ups to prove his shoulder is okay. That's more than what he got from the county doc.   • Today my patient called me a quack. In front of the entire office. I guess it was my fault for discovering that her claimed injury was not caused by her job. What was I supposed to do? Ignore the clinical notes in her chart and plainly see that she had been complaining of right shoulder pain long before the date she said she hurt herself at work? Or was I supposed to ignore the MRI of her shoulder that showed she had a chronic clavicular fracture in her right shoulder? Maybe I overstepped my bounds by taking an MRI of the left shoulder, which also showed a chronic clavicular fracture. It's quite possible I pushed her over the edge by asking about any prior injuries that would have caused bilateral clavicular fractures and a slew of degenerative changes in both shoulders. But here's the thing: I’m not a quack. I’m a physician skilled in determining injury causation. I aim to unclog the system from non-genuine cases, ensuring that truly injured individuals get the care they need.
