The theme of this week has been the power of partnerships. That extends from the health systems and physicians we partner with to bring Cadence to seniors battling chronic disease, to those who help us build, operationalize and grow Cadence. Cadence co-founder and General Partner at Thrive Capital Kareem Zaki is featured on this week’s episode of Patrick O'Shaughnessy, CFA's Invest Like the Best discussing his approach to investing in category defining companies, and how to solve some of the biggest problems in the healthcare industry. “We're not going to build outside the system. We're going to build in the system," he shares of the work Cadence is doing to improve healthcare’s infrastructure. A great listen for those interested in building and investing in the future of healthcare. https://lnkd.in/eehzui7H

The Nashville Health Care Council's #Sessions2024 discussion about the evolving needs of America's aging population included themes about creating access to the services seniors need, a greater focus on preventative care, building trust, and creating care models that intervene faster to address medical needs in the outpatient setting, preserving acute care for when hospitals are needed most. Community Health Systems CEO Tim Hingtgen noted that most of the one million patient encounters happening across CHS health systems each month are for outpatient care. He said that developing stronger patient-provider relationships and making it easier for seniors to connect to the various services they need are keys to better care and fewer hospitalizations for aging Americans. It was also the perfect time to talk about the benefit of CHS' at-home, remote patient monitoring and chronic care management program powered by Cadence. With more than 14,000 patients enrolled, CHS and Cadence are helping older adults live healthier, providing peace of mind for patients and their families, and we are achieving better outcomes. Yesterday at Sessions, Cadence CEO Chris Altchek also talked about how remote patient monitoring is a huge advantage for many people now, and will become an imperative in the future, if we are going to continue to help and improve the health of people living with chronic conditions like diabetes, hypertension, and heart failure. #betteroutcomes #livehealthier

This afternoon Cadence founder and CEO Chris Altchek joined Lifepoint Health®’s David DillAscension Saint Thomas' Fahad Tahir and TeamHealth’s Leif Murphy on stage at Nashville Health Care Council #Sessions2024. The group delved deep into the transformative power of collaboration in driving results for patients, communities, and health systems. Chris shared, “When it comes to these kinds of partnerships, instead of angling for ownership or interest, we take the approach of ‘we’re all in this together’ because it’s really not a zero-sum game.” Technology will have an even more profound impact on how healthcare is delivered in the coming decade. It’s not just about making care more efficient, it’s about making it more proficient. The future is about forming fewer but deeper partnerships, working alongside the best health systems to personalize guidelines and raise the standard of care at scale.

Today’s #MissionMonday spotlights a Cadence patient of two years, Marilee Keller from Marquette, Michigan. In a recent conversation with our Care Team she shares how helpful Cadence has been on her health journey:  “The people I’ve connected with over the last two years have been wonderful. I look forward to it,” Marilee said. “Having this to go to in between my visits is a real gem. We review these reports from Cadence every time I see [the PA]. Having the record of my blood pressure and weight loss on a daily basis helps me to relax and enjoy life as it is. This is a routine for me and something I will continue to do as long as I am able.” We are honored to support patients like Marilee, who find comfort and confidence in the consistent care and insights provided through Cadence. Her experience highlights the value of continuous health monitoring, enabling peace of mind and better management between in-person visits. Listen to her full story below!

At Cadence, we resonate deeply with Dr. Eve Cunningham MD MBA’s insights on the burdens primary care physicians face. In her recent fireside chat at the MedCity News INVEST Digital Health conference, Dr. Cunningham addressed the overwhelming influx of patient data, much of which burdens physicians with unnecessary details. She emphasized how the healthcare system is strained, with physicians often feeling they can’t take on new patients due to anxiety over missing critical information. Remote patient monitoring, especially for chronic conditions like congestive heart failure and diabetes, offers a solution. Dr. Cunningham explained, “Partnering in a meaningful way, being mindful of the impact that you’re going to have on the workforce that’s doing brick and mortar work, and so that it makes sense to them and to their patients.” At Cadence, we’re committed to easing this burden by seamlessly integrating into existing care teams, supporting both patients and healthcare providers. As the healthcare industry evolves, these efforts are crucial to alleviating burnout and creating a sustainable future for virtual care. Together, we can improve outcomes, reduce the workload on primary care, and bring the next generation of healthcare solutions forward. https://lnkd.in/ddRdSqh2
