Intraoperative hypotension can be damaging for patients of all ages. A multicenter retrospective cohort study of over 330K patients showed that there was an association between IOH and major adverse cardiac or cerebrovascular events (MACCE) for all age groups. https://lnkd.in/ecuivt59 A retrospective cohort study of 4,952 noncardiac surgery patients under 60 years old found that a MAP <55 mmHg for over 10 minutes was a serious risk factor for AKI. VitalStream is now available on the VA Federal Supply Schedule (VA FSS), a streamlined procurement method for the U.S Department of Veterans Affairs. VitalStream offers VA medical centers the ability to continuously and non-invasively monitor BP, CO, SV, fluid responsiveness and other advanced hemodynamic parameters throughout all hospital departments. This level of monitoring has been previously unavailable outside the OR and ICU, and is usually reserved for higher-risk patients and procedures since it involves invasive lines or uncomfortable and restrictive devices. When it comes to fluid administration, one size does not fit all. The SEP-1 guidelines are aligned with the International Guidelines for Management of Sepsis and Septic Shock which suggest that fluid administration be guided by a careful assessment of fluid responsiveness to avoid over and under resuscitation. As of January 1, 2024, SEP-1 performance has been added to the hospital VBP program under the Safety domain. V5.14 of the Hospital IQR Measure Specifications Manual added guidance to allow for clinical judgement and individualized fluid administration. Documentation of the CO, CI, SV or SVI used in the assessment is required if a lesser amount or no fluids are ordered. This was included because of the importance of fluid assessment on outcomes and cost of care, as well as the availability and accuracy of noninvasive technology such as VitalStream.
