This is a great article as to why retail has had a difficult time attracting the Senior Clientele. Clinics are just a piece of the Senior healthcare puzzle and the convenience of being able to "stop-by" is probably not enough of draw to engage this population. Especially when they have a myriad of health related issues that need to be addressed by multiple specialists. I agree with the comments made by Jeff Charles Goldsmith that "Convenient Care is for young people". That being said having access to a retail location such as Kroger Health that is convenient for those basic check-in and prescription refills has value but it will never replace the PCP and the relationship the client has with their Medical Professional.

After being in the aging care space for almost 30 years I can honestly say, in this past week, I have seen more comments and acknowledgement of what is clearly a broken delivery system than ever before. While it is a daunting task to rip a business model down to its foundation it is clear that is what needs to happen. I appreciate the comments by Ari Medoff and Mari Baxter who are looking forward to the future and Andrew Dolan at Home Health Care News for continuing to keep the conversation going.

Great article by Andrew Christman and Dianne Munevar from NORC at the University of Chicago. This segment of the aging population needs solutions that enable them to age where they want and not have to sacrifice care for food or lodging. I was saddened to see that Atria Senior Living elected to divest of its Homecare division, I knew many of the local team who were dedicated and committed to the clients they serviced. The cost and focus of operating a Homecare agency, especially in NY is hard to justify for Senior Living. The services provided by Homecare agencies are critical to the wellbeing and quality of life of the Senior Living resident and these services need to be offered in every community, this was one of the focuses for my team and I at CaringOnDemand. We provide residents access to regulated homecare services at the push of a button, supported by local regulated agencies. Our technology eliminates the care silos created by the Thousands of Homecare agencies and puts control back in the hands of the resident and their family members. This allows Senior Living to have a consistent care delivery brand presence across their entire porfolio and realize new revenue opportunities without the capital investment in building their own homecare agency.

In a vacuum I agree with Chip Kahn and the need for the Hospital system to protect its investment, which we all agree is not insignificant when recruiting, training, insuring as well as all the other tangible and intangible cost associated with their highly skilled professionals. However, we do not exist in a vacuum and if this highly skilled physician finds that they need to leave the hospital for a myriad of valid reasons they are bond via this non-compete agreement and in essence forced to stay, that seems very "Un-American". Iam curious if the Hospital would analyze and calculated the total amount of billable services associated to this particular Physician, and then determine if they (The Hospital) have realized an ROI on their investment? If they determined that they indeed profitted would they then release their employee from further obligations under their non-compete? What is a fair ROI to allow employees to take their skills to another institution?

I suspect this proposal is more to pander to the Senior demographic before an election. How this would get paid for via a system already poorly financed remains to be seen. Seeing this topic on the National stage is a positive development but as usual leaves us with more questions than answers.

While this article brings to light the challenges being experienced by the rural population the same issue persists even in metropolitan markets. The coordination of services post hospitalization leaves room for disruption. While filling the gaps in Medicare episodic coverage with supportive services is a viable option for the healthcare systems to consider and would significantly reduce the risk of rehospitalization while improving the clients quality of life.
