About us
WHO WE ARE CenterLight Healthcare PACE is a leading non-profit provider of health and related services to older adults in need across the New York metropolitan area. WHO WE SERVE CenterLight Healthcare PACE is designed for those with Medicare and/or Medicaid for participants aged 55 years and older who require long-term care. CenterLight Healthcare PACE serves participants from a numerous backgrounds and languages including Arabic, Albanian, Chinese, Creole, English, Korean, Hindi, Punjabi, Russian and Spanish. WHERE WE OPERATE We operate 11 PACE centers and one Alternative Care Setting (ACS) throughout the five boroughs of New York City, Westchester and Suffolk counties, where participants can receive on-site socialization, social and personal care services, therapeutic recreation, meals, and transportation. Our PACE centers also offer medical services, rehabilitation, prescription and over-the-counter (OTC) medications. THE CENTERLIGHT MISSION To provide individualized care that enriches the lives of those we serve and helps them live safely in their homes and communities. A TEAM-BASED APPROACH TO CARE CenterLight Healthcare PACE develops individualized care plans, which are carried out by a dedicated interdisciplinary team (IDT) of doctors, nurses, physical therapists, nutritionists, recreational therapists and other professionals working together to provide care as unique as the richly diverse participants enrolled in the plan. We’re committed to helping seniors access the care and support they need to live safely at home and in their communities for as long as possible. The event was designed to encourage seniors to stay active, socialize, and enjoy the many benefits of movement in a supportive environment and the instructor tailored the session to their needs. Our team, including our very own veteran Brian Romick, shared information about our Program for All-Inclusive Care for the Elderly (PACE), designed to help older adults maintain their independence. CenterLight is committed to serving the needs of our veterans, ensuring they have access to comprehensive care.
