In 1992, Washington’s low-cost clinics (known as Community Health Centers) realized too many people were being neglected by traditional insurance. They got together and started Community Health Plan of Washington. CHPW is the state’s first not-for-profit managed care plan founded by Community Health Centers. Today, we’re still the only not-for-profit to provide managed care to Apple Health members. Always working to improve our services, we expanded to cover Medicare patients in 2007. Eligibility Coordinator - Hybrid, WA CHPW is seeking a motivated individual to join our Customer Experience department as an Eligibility Coordinator! In this role, you will oversee the coordination of Washington Apple Health eligibility/enrollment process, managing tasks such as data verification, error correction, updates, disenrollment, and enrollment tracking. Additionally, you will be responsible for comprehending and administering the contract requirements for each CHPW product. Successful candidates will have a minimum of 2 years of experience in eligibility processes and procedures, be familiar with State Medicaid Programs, and have working knowledge of medical terminology. Health Homes Care Coordinator – Remote with required travel within Spokane County, WA CHPW is seeking a Health Homes Care Coordinator to join our Care Management team! This dynamic, field-based role requires travel throughout Spokane County. As a Health Home Care Coordinator, you will support designated CHPW members in creating and executing a Health Action Plan (HAP) aimed at enhancing their health, engagement, and overall quality of life. Ideal candidates will hold a current and unrestricted RN, LPN, or LMHC license, along with two years of experience working with individuals facing complex chronic mental health issues and/or substance use challenges. Clinical Care Coordinator (BSW or LPN): We are looking for a self-motivated and highly organized individual to join our care management team as a Clinical Care Coordinator! In this role, you will assist and coordinate case management activities under the direction of an RN or LICSW Case Manager, Supervisor, or Manager. You will function as an interdependent team member in the areas of member assessment, planning, facilitation, and advocacy. The goal is to improve member quality of life and ensure cost-effective outcomes by using internal and community-based resources. Successful candidates will have demonstrated organizational and time management skills, along with at least a year’s worth of experience in clinical and/or case management. Bilingual skills a plus. Community Engagement and Complex Case Manager I - Spokane County: Join us as a Regional Case Manager, reporting to the Manager of Case Management! You'll oversee regional care management programs and processes, providing comprehensive case management services for CHPW members with a range of medical and behavioral conditions.   • Our local CHPW team was honored to be part of the 2024 Latina Health Symposium on September 9, 2024, at Renton Technical College. This annual event is a forum for community health workers, public health professionals, Latino/a/e community members, and social service professionals to come together to learn about and discuss health-related topics that impact Hispanic/Latina women. As a sponsor of the annual event, hosted by the Fred Hutch Cancer Center Office of Community Outreach & Engagement in celebration of Hispanic Heritage Month, we had the privilege of sharing a few words with attendees. Reyna Santiago (CHPW Account Manager) delivered a passionate speech and shared the story of a CHPW member, originally from Columbia, who was experiencing multiple barriers to access to care and was just days away from giving birth. Reyna shared how one of our Community Health Workers was able to connect with the member and let her know she wasn’t alone. From making sure the member had groceries to come home to after having her baby, to ensuring she had assistance in enrolling in low-income utility programs, CHPW was with her every step of the way. "At CHPW, our overarching strategy is unwavering: to be a leader in the pursuit of whole-person care and health equity. This means recognizing that Latinas need equitable access to quality care and the best way to ensure we get there is to come together as a community. We’re all here today because we understand that together we can help address social drivers we know are greatly affecting Latinas across the state," Reyna shared during her speech. Last year, the National Women’s Law Center published the “Social Determinants of Health Metrics for Latinas by State” and as it turns out, in Washington State, approximately 24% of Latinas ages 19 to 64 lacked health insurance between 2017 and 2021; nationally, one in five Latinas lacked health insurance coverage during that time. Simply put, lack of health insurance coverage makes it more difficult for Latinas to access and afford health care. The health and well-being of Latinas begins with access to quality care. At CHPW, our work focuses on fostering equitable, whole-person care by ensuring that every member can access a thorough, individualized, and interconnected health care approach. We're committed to delivering accessible managed care services that meet the needs and improve the health of our communities and we're grateful for the opportunity to collaborate with providers and members in communities across Washington State.   • Thank you, Joel Martin and Columbia Basin Herald, for spotlighting Haziel Sandoval-Gatica who received CHPW’s Power of Community Perseverance Award at this year's annual Office of Superintendent of Public Instruction's 2024 State Conference Art of Belonging. We continue to be inspired by Haziel's drive and ambition. As a recent high school graduate, his future is bright and through the power of community, we have no doubt Haziel will achieve the goals he sets out to accomplish. CHPW Regional Manager Daniel Smith had the privilege of contributing to the article: “(Sandoval-Gatica’s story) just really resonated with our organization's commitment to do whatever we can to build partnerships, to empower young people and to remove barriers so that they can continue to excel despite (them),” Smith said. “Every obstacle that might seem like it's in front of them, and Haziel just stood out as a remarkable achievement. I mean, he had a choice. He didn't need to go back to school. He didn't need to put in all the work and do all the things that were necessary to be successful.   • We put on our running shoes and got our heart rates up for a great cause at Benton-Franklin Recovery Coalition's annual fundraiser, the 5K Run for Recovery on September 14, 2024, in Pasco, Washington! Joel Chavez, CHPW Regional Manager, made sure to stretch and hydrate before taking off to join the community to run for recovery. Recovery looks different for individuals but one thing is certain, educational organizations, like Benton Franklin Recovery Coalition, are essential to connect community members with community partners to help in the recovery process. We were honored to sponsor this fundraiser to support their work advocating for addiction treatment and recovery services. At CHPW, we believe working in collaboration with community partners is essential to respond to the specific needs of our members, remove barriers, and ensure quality care. National Recovery Month is celebrated in September to promote and support new treatment and recovery practices, the nation's strong and proud recovery community, and the dedication of service providers and communities who make recovery in all its forms possible. Benton Franklin Recovery Coalition advocates for modified practices in medicine, the legal system and other areas to address substance use disorder. In addition, they provide educational materials and hold meetings and forums to teach about addiction, and thereby remove stigma, shame, and secrecy from it. Behavioral Health Utilization Management Clinician I – 100% Remote in WA, OR, or ID CHPW has an exciting position becoming available on our Utilization Management team! As a Behavioral Health Utilization Management Clinician I, you will be responsible for medical and behavioral health requests using utilization review criteria, technologies, and tools. You will identify, coordinate, and implement high quality, cost-effective alternatives when appropriate to the patient’s condition.
