About us
Community Health & Wellness Partners was inspired to provide all members of our community with access to quality health and wellness services. Our doors are open to everyone with no economic barriers to care. We turn no one away, and we connect our patients to all the resources they need to enhance their lives and provide hope to get well and live life fully. Community Health & Wellness Partners was born as a federally chartered healthcare facility. Our goal is to touch and serve all residents -- those fighting addition, depression, in need of primary care, managing diseases, dealing with family matters, members of minority communities and refugees. We serve patients from birth to death. We connect with and are recognized by other social service agencies as having made an impact in the lives of our residents. Fifty-one percent of our board are patients living in Logan County and surrounding communities. We provide services to 25 percent of the Logan County census population. Every member of our team -- whether it is medical professionals or the staff who support all that we do -- embraces our patients and their families with smiles, genuine kindness and thoughtful care to help them get well regardless of the situation. We have a strong commitment to outstanding patient care. We give our patients access to all the resources they need to get well. Whether we are finding alternatives to insurance barriers, recommending in-house or other resources to ensure whole person wellness, educating patients on disease management, reviewing and regulating medications, or providing transportation to get them the care they need, patients have our full commitment to their care. Our commitment for care is whole person -- whole community.

Engaging the top of Ohio's future healthcare workforce! CHWP has partnered with @Ohio Hi-Point Career Center to host allied healthcare students in their Medical Assistant Residency Program. These students are employed by CHWP and receive 1:1 training by CHWP's Clinical Coordinator Registered Nurses in the areas of primary care and behavioral health. These students will graduate @Ohio Hi-Point Career Center with enough hours of medical assistant training in preparation to sit for their medical assistant testing.
