When partnerships and programs can be built between community-based organizations and ACOs that are designed to specifically address the social needs of food insecurity and housing instability for members, we know they work and reduce the cost of health care! Since Community Care Cooperative began its Flexible Services Program in collaboration with the state of Massachusetts in 2020, we have been able to help more than 13,000 members, with a range of services that include food vouchers and providing medically-tailored meals to diabetes patients, who, in the first six months after their enrollment in the program, saw their cost of care fall by $5,552 per person. We have also helped those experiencing homelessness find stable housing, which led to a dramatic decrease in the number of times they would visit the emergency departments at their local health center facilities. About us
Community Care Cooperative (C3) is a 501(c)(3) not-for-profit, Accountable Care Organization (ACO) governed by Federally Qualified Health Centers (FQHCs) in Massachusetts. In 2016, leaders from 15 FQHCs throughout the state formed C3. An innovative model for an Accountable Care Organization (ACO), C3 leverages the proven best practices of ACOs throughout the country. C3 is the only ACO in the state founded and governed by FQHCs exclusively focused on advancing integrated and coordinated community-based care for MassHealth members. C3 is building its organization on the collective strengths of health centers. We currently have 17 partner health centers across the state and our continued growth and expansion enables C3 to better serve MassHealth members across the Commonwealth.

The legend of Tiburcio Vasquez dates back to the late gold rush era of the 1800s—a tumultuous time in California. Vasquez was a bold, flamboyant risk-taker who won the hearts of many Spanish-speaking early Californians and Native Americans. Legends among the communities that loved him have painted a portrait of a romantic poet, a caring and giving man who often provided food and medical aid to the needy, and a passionate supporter of human rights. How can #Medicaid policymakers advance the adoption of value-based arrangements by Federally Qualified Health Centers? 1. Through the Return of Shared Savings to #FQHCS. – Participating FQHC networks should be eligible to share in a substantial portion (50% or more) of the savings realized when the actual total costs incurred for their attributed population are below the benchmark or goal set for average Total Cost Of Care (TCOC). 2. Adjust the Payment Model for the FQHC Patient Population. – The benchmark costs used to set rates should be based on the average #TCOC in the market, or the average amount the Managed Care Organization gets paid per enrollee, rather than using the FQHC networks’ own historical low reimbursement as a starting point.
