At Continuum Care Hospice, we provide terminally ill patients and their loved ones with the support and care they require to live their lives comfortably. Our skilled hospice team is dedicated to helping our patients and their families maintain their dignity and quality of life. Our goal is to provide physical, emotional, and spiritual comfort for the patient and to provide tools, information and support for the patient’s family so that they feel confident in the crucial decisions that come along with terminal illnesses. We provide ongoing care and support for our patients and their families for as long as it is needed.

Last week, we held two annual memorials to honor our team members - one in Oakland and one in Concord. The goal was to ensure that our families could attend a service close to their location. Attendees shared photos of their loved ones and crafted personalized river rocks as keepsakes in memory of those they have lost. Our dedicated chaplains, Leo and Mary, facilitated both memorials with compassion and care. #OurCareFamily

Happy World Occupational Therapy Day! The theme for this year is, “Occupational Therapy for All.” The theme reflects the important role of the profession in facilitating the ability of individuals, communities, and populations to participate in the activities that they want, need, or are expected to do in their daily lives. Occupational therapy is a vital part of hospice. This form of therapy is used to help those recovering from physical or mental illnesses. We work to encourage rehabilitation to help with certain activities that are necessary for daily life. OTs play a crucial role in the hospice care team. They help identify significant life roles and activities that provide some meaning to patients. They’ll talk to the patient to find out what they want to accomplish and what’s most important to them. They also consider the available support systems and resources they may have to achieve those goals. Thank you, Occupational Therapist, for all the hard work that you do each day!

During Spiritual Care Week, we celebrate the chaplains who serve not only as spiritual guides but also as counselors, listeners, and compassionate companions. Their roles extend far beyond religious care, as they wear many hats to provide support wherever it’s needed most. Mary was an ordained minister and a qualified chaplain. She had previously worked at the VA as a chaplain. She was working on her doctorate, was between church jobs and a friend who happened to be a hospice nurse said, we need a chaplain part time. She stayed part time for about a month, went full time and never looked back. She is a great contribution to the Continuum team, and we are grateful for her service to our patients. Their roles extend far beyond religious care, as they wear many hats to provide support wherever it’s needed most. Meet Leo who has been a part of our team for three years. Leo had been a Catholic Liturgist and ritual specialist when his sister passed away from breast cancer. He oversaw and performed his sister’s funeral. After his sister’s funeral, many asked him to perform other funerals. Getting certified took many years, but he knew he had chosen the right path. He did his rounds in the oncology department and just listened. He found his way to hospice and likes that he gets to listen to people. He likes that people can go gracefully, with ease and comfort. He is very caring, and kind and we are lucky to have him as a part of our team. #SpiritualCareWeek #MoreThanReligion #ManyHatsOfCare #OurCareFamily
