Expert Wound and Ostomy Care. Anywhere.
About us
EXPERT WOUND & OSTOMY CARE. ANYWHERE. We bring expert wound & ostomy care to the patient...wherever they are...from hospital to home and everywhere in between. Our experienced, board-certified wound clinicians work with healthcare providers, payers, and patients to provide best in class wound care at a lower cost. Our board-certified wound & ostomy clinicians use both live video and wound images to evaluate a patient’s wound and consult with the provider to recommend the most effective treatment for better outcomes, improved patient quality of life, and lower overall cost. Our mobile app allows a patient, a caregiver, or an onsite clinician to easily take wound photos, document the status of the wound and initiate a live video call with Corstrata’s wound experts. Through our cloud-based telehealth platform, our experts monitor the wound’s progress and recommend treatments that promote healing. Corstrata’s HIPAA compliant technologies facilitate documentation which reduces the provider’s legal liability. We help healthcare providers become Centers of Excellence for wound care by partnering with the provider for their wound care program. Our services range from comprehensive wound care management to a menu of services to enhance your current program: Wound program assessment Staff education & annual competencies Formulary review & assessment Consultations for complex wounds Pressure injury prevention program Diabetes foot ulcer prevention & management program Hospice & palliative wound management program Ostomy management & education Documentation review for compliance & defensibility We help payers case manage patients with complex wounds, promoting healing and prevention of wound recurrence. We also facilitate management of the wound patient as they transition from one care setting to another- often the time when the patient is most vulnerable to inconsistent, poorly coordinated care.

What I often hear from my colleagues is that dehydration is a primary culprit as patients avoid adequate intake over concerns of leakage. Post discharge monitoring and support can help patients navigate those first few weeks and gain the confidence to assume life as before - only better!
High readmission rates following ileostomy surgery are often due to dehydration, complications with stoma care, and a lack of proper patient education. Research shows that providing timely expert support can significantly reduce these rates. By connecting patients to ostomy specialists, healthcare providers can address issues early on, improving outcomes and reducing costly readmissions. #WoundCare #OstomyCare #HealthcareInnovation #ReadmissionPrevention

A Personal Mission in Wound Care The memory of my father’s last months, burdened by #chronicwounds and unbearable #pain, drives my dedication to revolutionizing #woundcare. Witnessing his suffering, exacerbated by gaps in specialized care, showed me the profound impact of inadequate treatment. It was not just a medical failure; it was a failure to protect his dignity in his final days. 💔 This deeply personal experience revealed a widespread issue: there simply aren't enough wound care specialists to meet the needs outside of hospital settings. Many patients, like my father, endure unnecessary suffering because the expertise isn’t accessible where it’s most needed—in care settings like nursing homes, home health care, and hospice and in rural and urban #caredeserts. Corstrata was born from this urgency to do better. We use #technology to connect bedside nurses with wound care experts, enhancing the quality of care through #virtualconsultations. This means more patients can receive the specialized attention they need, significantly improving their quality of life. This work is more than a professional endeavor; it's a commitment to change the narrative of #woundcare. By ensuring others don’t face the same struggles as my father, I honor his memory and move closer to a future where effective wound care is within everyone’s reach. I look forward to sharing my story with other NurseApproved companies at the #HLTH2024 conference next month. #WoundCare #HealthTech #PatientDignity #NurseApproved #HLTH2024
