As a young adult, Justin experienced an issue with his blood that led to hospitalization, the need for higher levels of care, and dialysis. As his DaVita care team started working with him and his family, they focused on building trust and creating a strong connection with him. His coordinated care helped him maintain eligibility for kidney transplantation. After months of successful treatment at DaVita, Justin celebrated receiving a kidney transplant in February 2024. “Access to quality health care should not be a luxury. Rather, it should be readily available in every community.” This is the belief that guides our 2024 Health Tour as it embarks on a nationwide journey to provide free health screenings and chronic disease education to communities in need. Over the next two months, the DaVita Health Tour will travel across five states to offer simple, and often transformative health assessments so individuals feel empowered to stay in control of their health. When communities have limited access to care, declining health can go undiagnosed until reaching advanced stages. For chronic kidney disease (CKD), a progressive condition that often develops silently until reaching kidney failure, this is particularly urgent. Dr. David Roer MD FACP FASN, chief medical officer emeritus for DaVita IKC, recently joined Dr. Nathan Moore of BJC on stage at The Health Management Academy 2024 to discuss how defragmentation of care in complex patients with end-stage kidney disease (ESKD) can improve clinical outcomes. This discussion touched on the value of integrating these concepts into provider-to-provider value-based structures. The DaVita Health Tour is back this fall! Beginning September 24th, our mobile health clinic is hitting the road for a third year to deliver preventive care to communities across the nation. We’re providing free health screenings and chronic care education at neighborhood events to equip community members to take control of their health and detect and manage conditions that can lead to chronic diseases, such as chronic kidney disease (CKD) before they progress. With 1 in 7 adults in the U.S. living with CKD, and most unaware of it, it's critical to improve access to health screenings and strengthen kidney health awareness and education. Nursing schools historically include minimal focus on nephrology, but we're working to change that. We're focused on securing collaborations with universities so we can increase understanding of how uniquely rewarding the #nephrology specialty can be. In an effort to make nephrology curriculums more accessible, we will continue to invest in academic practice partnerships, offer clinical internships to nursing students, and grow our residency program for newly hired nurse graduates to get the support they need during their first year. I had the great pleasure of connecting with Renee Thompson DNP, RN, FAAN, Workplace Bullying Expert, my friend and host of the Healthy Workforce Institute’s “Coffee Break” podcast, for a conversation about building strong teams. As an expert on civility in the workplace, I loved hearing Renee’s perspective and sharing my own take based upon the many roles at different levels on diverse teams I’ve held at DaVita Kidney Care.
