    Case Study: Over-Retained Milk Tooth and Impacted Canine in a 25-Year-Old Female A 25-year-old female patient visited our clinic Dr. Gul's Dental Clinic today, extremely anxious about a loose tooth in the lower front region. She was particularly worried about why her permanent tooth would be mobile at such a young age. **Clinical Examination:** On examination, I found that the tooth in question was not a permanent one, but an over-retained primary tooth, **tooth 73**. The patient was surprised, as she believed her permanent tooth had erupted long ago. **Radiographic Findings:** To investigate further, we took an X-ray, which revealed an impacted permanent canine, **tooth 33**, positioned toward the lingual side of the jaw. It had never erupted, leaving a noticeable, pea-sized bulge. The patient was visibly relieved when I explained the situation. Her words, *“Mujhe laga yeh permanent tooth hil raha hoga, par yeh to milk tooth hai. Permanent aya hi nahi!”* captured her disbelief. She hadn’t realized that her permanent tooth was still impacted, and what she assumed was her permanent tooth was actually a milk tooth that had stayed in place for far too long. **Treatment Plan:** We advised her to have the over-retained **tooth 73** extracted, followed by aligner therapy to guide the impacted **canine (tooth 33)** into its correct position. This canine is essential for a balanced bite and proper aesthetics, so timely intervention is crucial. **Key Takeaway for Fellow Dentists:** Over-retained milk teeth and impacted canines can easily be overlooked if regular checkups are missed, even into adulthood.     Today, we had a 35-year-old patient at Dr. Gul’s Dental Clinic who had been enduring severe pain that impacted his daily life for quite some time. He had previously undergone a root canal treatment (RCT) for his wisdom tooth (48), but unfortunately, it didn’t alleviate his pain. Upon examining his tooth, we found that most of the coronal portion was broken, and the best course of action was to extract the tooth. Within just five minutes, we carefully removed the problematic tooth. What truly touched us was the patient’s reaction—he was surprised at how quick and painless the procedure was and expressed immense relief. After so long in discomfort, he was finally pain-free.
