HCDI offers various services to help you and your family with health insurance enrollment and post-enrollment services. See below for descriptions of the programs. NYS Certified Navigators: HCDI staff complete applications for people in the Capital Region for Medicaid, Child Health Plus & Qualified Health Plans through the NYS of Health Marketplace. Aging, Blind and Disabled: HCDI staff assists in the application process for seniors, disabled and the blind for Medicaid, Medicare Savings Program, Spend Down, Nursing home and other programs. Albany, Saratoga, Schenectady and Schoharie County. Certified Application Counselor (CACs): Provides information about options and assistance with health insurance applications through the NY State of Health Marketplace. They meet applicants at convenient locations throughout the St. Peter’s Health Partner Network and Whitney M. Young Health Services. Community Health Advocate (CHA): Free assistance with medical services, and prescriptions and health insurance. CHA helps with disputes related to medical insurance, medical care or bills. CHA is here to help with any needs or resources. Patient Activation Measure Coaching: Provides a range of access to care and self-management support for newly insured consumers. Provide information on free preventive care, discount drug programs, disease-specific self-management, promote healthy behaviors, assist with problem-solving, provide regular follow up and encourage people to be active participants in their care
