We're excited to share that these case studies were awarded with Wayfinder Recognition which is voted on by peers to celebrate the successful partnerships that best demonstrated an overall positive impact while maintaining scalability, replicability, and sustainability. Falls are the most common adverse event in hospitals, and in 2023, four Veterans Affairs Medical Centers on the West Coast decided to take action. Partnering with Get Well, they implemented a proactive fall prevention strategy using digital engagement to empower veterans in their own care. The results? 📉 > 48% reduction in falls at one facility > $5.82 million in cost savings across four VA Medical Centers From "Call, Don't Fall" daily reminders to evidence-based educational modules, the comprehensive approach is changing outcomes for our veterans and setting a standard for patient safety. We're thrilled to share that Get Well has been recognized by KLAS Research for addressing social determinants of health within two unique partnerships. Representatives from these partnerships will present their case studies at the KLAS Social Determinants of Health tomorrow in Salt Lake City! 1️⃣ LifeBridge Health & Findhelp Partnership: Together, we tackled disparities in maternal health by leveraging technology to communicate effectively with patients. The initiative resulted in a 23% reduction in preterm deliveries and a 41% decrease in the length of NICU stays, demonstrating the power of personalized care. 2️⃣ Magnolia Health Plan Inc. & Canopy Children's Solutions: Focusing on youth mental health, this collaboration will bring a proactive program aimed at addressing social determinants and behavioral health conditions. The Canopy Anywhere pilot in Mississippi achieved an impressive 81% activation rate and we identified unmet needs for mental health support that had previously gone unnoticed. These initiatives underscore our commitment to redefining healthcare delivery through technology and community resources.
