About us
The Greater Hamilton Health Network (GHHN) is one of the first Ontario Health Teams across the province, with a purpose of transforming healthcare in partnership with patients, families, care partners, primary care, local organizations and the community. A collaboration of local health and social service partners the GHHN includes representation from more than 30 organizations, we are a dedicated group of professionals, organizations and people with lived experience working to co-design a patient centered, health system grounded in engagement, health equity and the local needs of the communities we serve.

PCN aims to improve the delivery and coordination of care for patients by connecting and supporting primary care providers and the primary care community so that we may thrive together within our local Ontario Health Team, the GHHN.

The GHHN was excited to attend la Plénière régionale de la tournée de sensibilisation sur l'offre, l'utilisation et la demande actives des services de santé en français (Regional Plenary of the Outreach Tour on Active Offer, Use, and Demand of French Language Health Services). Co-hosted by Entité2, the French language health planning entity covering Hamilton, the Regional Plenary was an incredible opportunity to talk about the barriers and opportunities for advancing access to and active offer of French-language service in our region. Also featured: a surprise chance to spend the day with the fantastic team at Thrive Group's long-term care facility at Idlewyld Manor!

The Social Navigator Program are still looking for adult winter coats, toques, socks, scarves, boots, and Mitts/gloves. This weekend, GHHN Staff member Marijke Ljogar, GHHN Patient Advisor Su Heenan, and the Urban Core team gathered for tree planting at Urban Core's new site! They planted four new trees: a Red Maple, two Burr Oaks, and one Serviceberry. Planting trees not only helps cool neighborhood temperatures and improves air quality, but it also brings the benefits of nature to our urban spaces, positively impacting mental health!
