This innovative Medicare alternative payment model is designed to enhance care for people living with dementia and support their caregivers across the U.S. By partnering with CMS, we aim to transform dementia care, improve access to crucial services like respite care, and provide much-needed support for caregivers through our Dementia Care Programs in 12 states (more coming soon).
