If left unmanaged, a patient’s diabetes can lead to very serious complications over time, including blindness, heart disease, and chronic kidney disease. For those who’ve been recently diagnosed with diabetes, Healthmap Solutions offers the guide “Diabetes: Preventing Complications.” This Patient Resource is available for download now in English or Spanish. Healthmap Solutions advocates for in-home dialysis options, when appropriate, for end-stage renal disease patients. Healthmap Care Navigators educate our program members on the advantages of in-home dialysis over in-center treatment, including greater convenience, flexibility, and comfort, all of which can lead to better clinical outcomes. Among members in Healthmap’s Kidney Population Health Management program, 20% to 40% of new dialysis starts utilize home dialysis modalities. That’s substantially higher than the national average of 13.3% reported by the United States Renal Data System! To learn more about our success in increasing home dialysis use, visit the link below. <https://lnkd.in/dWD2pi9Q>

Staying hydrated is essential for maintaining kidney health! Proper hydration helps your kidneys function efficiently by flushing out toxins and waste products from your blood. Aim to drink at least eight glasses of water per day (more if you’re very active or live in a hot climate). Not only does this support your kidneys, but it also promotes overall well-being. The National Kidney Foundation has more on healthy hydration here: https://hubs.li/Q02KbghF0

Effective care coordination and transition planning are essential to improving patient outcomes and eliminating unnecessary healthcare expenditures. This is especially true in the management of chronic kidney disease and end-stage renal disease. The complex, long-term nature of kidney disease requires consistent communication and collaboration among various providers. We cover this topic and strategies to improve care coordination and transitions in the latest Healthmap Solutions White Paper, “Care Coordination and Care Transitions In Value-based Kidney Disease Management.” Download it now from the link below: <<https://lnkd.in/dxYK-zCG>>

Healthmap Solutions Kidney Health Tip: Watch Your Protein Consumption While protein is essential to a healthy diet, too much protein can lead to kidney problems. Excessive protein can injure the kidneys by increasing waste production and filtration workload. For people with kidney disease, eating less protein may help slow the progression of their disease. Talk to your doctor or a registered dietitian to determine the amount of protein that will meet your nutritional needs and keep your kidneys healthy.
