We are proud to be one of only two Medicaid health plans in California that earned 4.5 Out of 5 Stars from the National Committee for Quality Assurance (NCQA) in 2024! This distinction reflects HPSM’s focus on our member’s health outcomes, and our continuous quality improvement efforts in many areas over time. Some of the efforts that contributed to our high-quality health outcomes for our members include: • Prenatal and Postpartum Care: Our Baby+Me Program helps pregnant members get access to timely prenatal care and postpartum care, through member outreach and incentives. • Diabetes management interventions: HPSM programs include an ongoing focus on promoting medication adherence and self-management programs, as well as providing members with case management and extra support during transitions of care. • Provider Incentive Programs: HPSM’s value-based payment programs are designed in collaboration with our providers, and are regularly revised to focus on closing important gaps in care. Our current  programs include incentives for providers to close care gaps in several priority areas, including: breast and cervical cancer screenings, child and adolescent immunizations, and providing members with support managing diabetes and high blood pressure.

We are always learning ways to serve our members better! One way we do this is with internal training and development. Staff from various departments became certified trainers after taking a recent three-day course with Global Learning Partners. This enables them to create customized employee learning experiences that engage both the head and heart.
