  • We can't emphasize enough the negative consequences of pharmacy benefit managers (PBMs). Please ask your local politicians to support legislation that will shine a light on PBMs corrupt practices and force them to show how they are pocketing rebates to benefit their bottom lines rather than improving health outcomes.

    “The PBM-mail-order pharmacy system focuses on profit and not the patient. Our cancer patients deserve better.” Pharmacy benefit managers (PBMs) are middlemen who interfere with cancer care year after year. In this PBM horror story, a 60-year-old with stage IV colon cancer was trying a third cancer therapy prescribed by his doctor. The PBM’s pharmacy demanded a $5,000 upfront payment, which would have been $0 with a copay card. Rather than helping him sign up for the copay card program, the pharmacy gave him the phone number and told him to call back after he registered. The patient contacted his community oncology practice, which helped him navigate the process and successfully change his copay to $0.   • We are proud to announce we are the first cancer care center in New York State offering Bispecific Antibody Therapy on an outpatient basis - eliminating the need for hospital stays! Prior to HOA’s program, the risk of complications from bispecific therapy’s initial dosing period required a patient be admitted to the hospital for administration of the medication and monitoring of reactions which many times do not occur. The hospital stay for bispecific therapy can range from a day or two on multiple occasions up to as long as 14 days. “Our program includes a very sophisticated outpatient monitoring plan that allows our patients the freedom to stay at home, avoiding potential infection risks, costs associated with a hospital stay, and time away from family,” explains medical oncologist Steven Duffy, M.D., Physician Champion for the Bispecific Therapy Program and HOA’s Director of Research. “Improving our patients’ quality of life during treatment for cancer and hematologic conditions is the impetus for this new program. We want to embrace these exciting new therapies so that people can get the best treatments close to home.” What's a Bispecific Antibody you ask? It's a new therapeutic agent that binds to two different cells – the disease cells and t-cells (a type of white blood cell that protects the body from infection and fights cancer) using the body’s own immune system to fight the disease. This therapy is a powerful treatment option for patients with liquid and solid tumors.   • We are happy to announce Cherie Tan, M.D. has joined our physician team, bringing our total number of physician oncology experts to 20! Dr. Tan is a medical oncologist and hematologist who completed fellowship training at the University of Maryland Medical Center - Greenebaum Comprehensive Cancer Center, where she was Chief Fellow. She will be working in our new Camillus Center. Currently, Dr. Tan is seeing patients in our Brittonfield location until our Onondaga Hill office relocates to Camillus on Monday, September 16.   • HOA employees are extremely generous and caring, and that starts with our physicians! Yesterday was the annual shaving of Dr. Richard Cherny’s head as part of his fundraising efforts for the Pan-Mass Challenge! He raised more than $6,000; to thank the staff for their help he ordered in an ice cream truck. Of course, what was most exciting was being able to celebrate with his two patients who have both been treated at Dana-Farber Cancer Institute. Lots of folks joined in the festivities, including Dr. C’s fundraising committee, his bake sale phenom (and patient) Diane, as well as Dr. Cherny’s family. Thank you Dr. Cherny for your commitment to helping others. Good luck this weekend on your ride. We’re so proud of you!

  • Thank you to #CommunityOncologyAlliance for this important review. COA believes that the findings of this analysis exemplify how the 340B program has increasingly been taken over by for-profit PBMs. When this happens, the PBMs retain significant portions of the profits. Previous estimates have found that for-profit PBM contract pharmacies retain 25% to 35% of the total 340B discount and have an average gross margin of 72%. This significant retention of profits is concerning, particularly as the 340B program is designed to support non-profit health care. PBMs have exploited the 340B program's lack of transparency, oversight, and accountability to siphon significant resources away from patient care, using the pharmacies they own or are affiliated with.

    Today, Avalere released an analysis that examines contract pharmacies within the 340B Drug Pricing Program. Specifically, the analysis examines vertical integration and contractual affiliations with pharmacy benefit managers (PBMs), mail-order, and specialty pharmacy contract pharmacy relationships in 340B.
