What a great, simple way to boil this down!
CISOs are wasting their time in the wrong places. Everyone knows the Risk Nine-Box: judging Damage & Likelihood by High/Medium/Low, and trying (and failing) to use it to prioritize work. Here's my realization: the four corners of the Nine-Box represent VASTLY different processes for cleaning up those problems. Low Damage/Low Likelihood? That's Litter - you should have a street-sweeping process that deals with most of this. Minor software defects should get cleaned up by CI/CD processes that (don't actually) keep software libraries relatively up-to-date. But there's no reason to count every piece of litter to complain that there isn't a street-sweeper. Low Damage/High Likelihood? That's Hygiene - yes, you should brush your teeth, and you should have routine processes that address interesting defects; your release process should pick up every security defect and get them scheduled post-haste. But we're not the parents who should be on overwatch to make sure teeth get brushed every night. High Damage/High Likelihood? These are Incidents, and they (mostly) take care of themselves. The smoke alarm goes off, it's all-hand on deck, and we generally don't have to tell people to prioritize these. All of those should have robust *processes* to ensure they happen; if they don't, the problem isn't the pile of litter or hygiene; the problem is the lack of mature processes in the organization. High Damage/Low Likelihood? That's the domain of security Programs - projects to make sure those other processes exist, or far-reaching rearchitecting of technology, processes, or organizations to mitigate complex risk. 99% of the issues that security teams focus on *aren't* in this category -- but they should be. Security teams spend all their time nagging other teams to do their jobs safely – are we the Karens of the organization? – instead of challenging organizations to behave appropriately.
