And we're back... We took a bit of a break in our production schedule but we just released the latest episode of Centering Health Equity. In this episode of Centering Health Equity, we are joined by Dr. Michael Sobolevsky , a podiatrist specializing in diabetic care at University Health’s Texas Diabetes Institute, and Dr Prieto, Francisco, a family physician and member of the Board of Directors for the American Diabetes Association and Chair of their National Advocacy Committee. Our conversation discusses the inequities surrounding Diabetes–a chronic condition that affects almost 15% of adults in the US and its prevalence among minority populations which is even greater. We cover the interventions in practice, the social drivers of health that impact the incidence of diabetes, and where there needs to be more targeted focus to reduce the debilitating impact of uncontrolled diabetes for these communities. Navigating Your Health Equity Playbook in 2024 We are extremely proud to share that Impact4Health is now just one of three organizations nationwide selected by the American Hospital Association to be Equity Transformation Partners.  In collaboration with AHA's Institute for Diversity and Health Equity (IFDHE), we are ready to assist health systems working to complete the Health Equity Roadmap and help their leaders meet new standards that are required by The Joint Commission and the Centers for Medicare and Medicaid Services.  There are four primary areas of work necessary to meet those standards: 1. Build the infrastructure necessary to advance health equity 2. Improve and track population health outcomes across demographic groups 3. Engage with communities served in culturally responsive ways 4. Attend to provider and employee development and sense of belonging Completing the Health Equity Roadmap is an excellent way to identify key best practices for adoption that lead to complying with these new standards. Taken together, these best practices can shift the culture of healthcare systems to be more inclusive of their workforce and responsive to the communities they serve.  In a recent report by the Rise to Health Coalition, leaders suggest that we are a decade away from achieving health equity.  We believe it's possible to move the needle faster!  Please join us for a free, live interactive seminar dedicated to reviewing the AHA Health Equity Roadmap and answering your questions about getting started.
