A recent article in the The New York Times reviewed research which found that online therapy has exacerbated disparities in mental health utilization and failed to expand access to underserved populations. Though this finding may be disheartening, we’ve conducted research at Included Health that paints a more nuanced picture. A 2024 study my colleagues Dr. Ami Parekh and Jaclyn Karlen Marshall conducted with the Urban Institute found that patients with frequent visits (5 or more/year) were more likely to be younger, female, Black or Other races, and reside in zip codes with greater levels of poverty. This same study found that the five most common diagnoses among frequent users were all mental health conditions — major depressive disorder, reaction to severe stress and adjustment disorders, other anxiety disorders, depressive disorder, and bipolar disorder, even though we offer comprehensive care including primary, specialty, and urgent care. More broadly, when we looked at self-reported data from Included Health patients who provided information on race and ethnicity, almost half identified as Non-white or Hispanic; of those who provided data on sexual orientation or gender identity, 27% identified as lesbian, gay, bisexual, asexual, pansexual, or queer and 8% identified as transgender, genderfluid, agender, or nonbinary. But, the financial picture matters as well. A study published in JAMA, Journal of the American Medical Association that we conducted with Harvard University found that cost-sharing impacts utilization; when current telehealth patients were required to start paying out-of-pocket, they had substantially fewer visits, and a larger fraction stopped seeing their mental health specialists. Because the exemption for virtual care for high-deductible health plans expired on December 31, 2024, we may see a decline in virtual visits among economically disadvantaged groups. Even though our research shows bright spots in terms of telehealth utilization, as a field we need to do more to ensure telehealth fulfills its promise. Here’s where we should focus: 🌍 Ensure care is high quality and impactful by hiring a diverse workforce and tailoring care; empirically supported methodology is not enough–care needs to incorporate cultural beliefs and values. 🗺️ Offer virtual and in-person services, as well as navigation support, to get people care for their specific needs. 🤝 Offer a holistic suite of services to better detect and meet mental health needs, since many individuals first seek mental health support through primary, urgent, or specialty care. 👷♀️ Partner not just with health plans, but with employers, since 54% of Americans depend on employer-sponsored coverage to cover health care costs. 📱Provide support with advocacy, billing, and claims adjudication, in order to ensure that patients are supported across the board–mind, body and wallet. 💰Advocate for policy that removes financial barriers like cost-sharing.
