In honor of American Pharmacists Month, we are celebrating our amazing pharmacists on staff. That includes new FDA/ISMP Safe Medication Management Fellow Desire’ Johnson. Desire decided on her career path after volunteering at a retail pharmacy, where she was impressed by how much the pharmacists cared about the entire community. She received her undergraduate and doctorate in pharmacy from Mercer University and completed a PGY-1 residency at AdventHealth Altamonte Springs in Florida. She has worked as a clinical pharmacist in multiple areas, such as pediatrics, emergency medicine, and sterile compounding. One of her most memorable experiences was a regulatory pharmacy rotation with the Georgia Department of Public Health during the pandemic, which included answering questions posed to the state’s COVID hotline. ISMP appreciates Desire' and all the other dedicated pharmacists who help keep patients safe every day! #medicationsafety #medsafety #patientsafety #medicationerrors #pharmacist

Advancing patient safety worldwide by empowering the healthcare community to prevent medication errors.
About us
The Institute for Safe Medication Practices (ISMP) is the only 501c (3) nonprofit organization devoted entirely to preventing medication errors. ISMP is known and respected as the gold standard for medication safety information. For more than 30 years, it also has served as a vital force for progress. ISMP’s advocacy work alone has resulted in numerous necessary changes in clinical practice, public policy, and drug labeling and packaging. Among its many initiatives, ISMP runs the only national voluntary practitioner medication error reporting program, publishes newsletters with real-time error information read and trusted throughout the global healthcare community, and offers a wide range of unique educational programs, tools, and guidelines. In 2020, ISMP formally affiliated with ECRI to create one of the largest healthcare quality and safety institutions in the world. As a watchdog organization, ISMP receives no advertising revenue and depends entirely on charitable donations, educational grants, newsletter subscriptions, and volunteer efforts to pursue its life-saving work. Christina Marucci Michalek and Jennifer Young recently presented an interactive session at the National Association of Specialty Pharmacy (NASP) Annual Meeting. They provided a review of three top medication safety concerns, including multi-tiered strategies for preventing errors, recommendations for creating safer medication prescription labels, and alternative solutions to “benchmarking” error rates. The slide shown in the photo was part of a story illustrating how important it is to pay attention to small details! As healthcare organizations work to manage fluid shortages following manufacturing plant closures due to damage from Hurricane Helene, ISMP is providing recommendations for safe practices to reduce the risk of errors. The most recent issue of our acute care newsletter provides a list of strategies to follow, including evaluating all systems impacted by workflow or product changes, using IV workflow management systems in the pharmacy when manipulating or compounding products, and conducting a review when any new product is procured to identify potential risks with design or look-alike labeling and packaging: https://hubs.la/Q02TYcNJ0 #medicationsafety #medsafety #patientsafety #medicationerrors #hospitals #pharmacist #nurses #helene #intravenous ECRI Baxter International Inc.

Though immunization is one of the greatest public health achievements, continued success relies on the quality with which vaccines are prescribed, dispensed, stored, and administered. The 2022-2023 Bi-Annual Report from the ISMP National Vaccine Errors Reporting Program analyzes data on causes and contributing factors for vaccine-related events, discusses possible contributing factors, and provides effective risk reduction strategies: https://hubs.la/Q02TR5dv0 #medicationsafety #medsafety #patientsafety #medicationerrors #hospitals #vaccination #vaccine ECRI Centers for Disease Control and Prevention

ISMP offers deepest sympathies to those impacted by Hurricane Helene and is working to support healthcare practitioners and organizations in affected communities. A Baxter International Inc. facility in North Carolina has temporarily halted production due to damage and provided a list of impacted products, including several IV fluids. We encourage organizations to evaluate their current inventory and clinical needs, optimize fluid management, and identify alternatives to use when possible. ISMP’s affiliate, ECRI has released crucial guidance for navigating supply chain interruptions due to the hurricane: https://hubs.la/Q02S1PZx0 #medicationsafety #medsafety #patientsafety #medicationerrors #hospitals #pharmacist #nurses #helene #intravenous
