Identifying novel biomarkers with TMS-EEG – Methodological possibilities and challenges TMS-EEG is an up-and-coming method for biomarker identification. In theory, the biomarkers can be obtained from any neocortical brain area. However, it might be challenging in practice, and we are still lacking evidence of this as most of the studies have been limited to a few cortical areas. TMS-EEG can potentially also identify biomarkers unattainable with other methods. This is because TMS can activate both the GABAergic inhibitory and glutamatergic excitatory systems, but differently, and these different activations can be recorded with EEG. TMS-EEG can also index distinct GABAA and GABAB physiologic activity based on the fact that these indices have different timescales that are consistent with their underlying neurotransmitter physiological properties. The TMS-EEG biomarker field is still somewhat limited as more work is needed to unravel some of the neural origins of measures and understand the interaction between neural signals and artifacts. Furthermore, most protocols have not been studied for their repeatability yet with TMS-EEG, and with many protocols, we may still need to optimize how to measure each protocol. Our findings are best explained by sleep spindles exerting an asymmetrical “pulsed inhibition” on corticospinal excitability, complementing previous work demonstrating a phasic modulation of corticospinal excitability by the sleep SO (Bergmann et al. 2012) as well as asymmetrical “pulsed facilitation” by the sensorimotor mu-alpha rhythm during wakefulness (Bergmann et al. 2019; Zrenner et al. 2018). Utilizing this technique, future work should further characterize the excitation-inhibition profile of sleep spindles, investigate their interaction with SOs, and test their role as windows of phase-dependent synaptic plasticity as well as their causal relevance for memory reactivation in the context of systems memory consolidation. These advancements are backed by robust clinical trials and real-world data that validate the efficacy, safety, and increasing versatility of TMS therapy. 1. Time-Efficient Treatment Protocols Recent FDA clearances have introduced time-efficient TMS protocols that significantly reduce the duration of individual sessions. Intermittent Theta Burst Stimulation (iTBS) has emerged as a notable innovation, with treatment times reduced from 30-40 minutes to just over three minutes per session. Research has confirmed that these shorter protocols maintain the effectiveness of traditional TMS therapy, making it more accessible for both patients and clinics.
