    DePuy Synthes CMF had a wonderful experience at Lifenet Health with Dr. Richard Tyrell, M.D, Dr. Eva Dentcheva, MD from the Center for Plastic Surgery at Sentara Fort Norfolk working to provide the plastic surgery residents the opportunity for in depth learning about managing Craniofacial trauma this past weekend. Dr. David Powers, MD, DMD, FACS started off the day with an excellent presentation and then the learners spent the day in the lab practicing and perfecting their techniques for managing facial trauma. We were treated to a biomaterials lecture and tour of the Lifenet facility as well. It was a day full of learning. Thank you to our local team Brian Jones and Thomas Markley for all of their hard work and effort in making this a success.     “Giving healing every advantage" isn’t just a phrase that we use loosely. Our employees truly personify that message. Shelby Giblin, associate product manager for our cardiac and vascular solutions, lovingly donated her placenta after giving birth to her beautiful baby boy. The gift of birth tissue can help 75 or more individuals. Those impacted by severe wounds, traumatic burns or those undergoing surgical procedures such as dental or sports medicine. Birth tissue can also be used for research projects dedicated to better understanding diseases, such as cancer, and how to treat them.     Improving efficiency and optimizing patient care using the most current advances in Orthopaedic Trauma and Arthroplasty

    Patient is a 68-year-old well-controlled diabetic who sustained a fall from ladder approximately five months prior. Patient initially underwent staged fixation of pilon fracture and progressed towards painless motion of ankle around 3 to 4 months. Unfortunately, around month 5 with increased activity, patient began having worsening ankle pain and begin having to take anti-inflammatories as well as Tramadol again in order to get through his day. After discussion with patient in regards of risk and benefits of bracing and other conservative therapies, he choose to proceed with tibiotalar fusion given normal preop infection labs. Patient has a significantly small body habitus with areas of thin post surgical skin as well as minimal fat and tissue excursion. Based on this, we chose to proceed forward with implant removal and tibiofusion using suprapatellar nailing as opposed to traditional plate or TCC nailing as patient did not wish to lose any motion in subtalar joint if possible. We began with small strategic incisions to remove all implants which we felt would be in the way. We did a full removal of articular cartilage in the tibiotalar joint, as well as lateral gutter and syndesmosis. In order to decrease the overall size of patient’s ankle, we performed a medial mal osteotomy, and then this was used with vivagen to backfill the lateral avascular necrosis void. Following this a guide wire was placed in appropriate position utilizing the reduction finger to help guide placement. Following placement of nail, two headless screws were placed in the talus and gentle back slapping allowed for compression. A screw was placed proximally and 5 mm of additional compression was gained through the proximal reduction tool followed by screw placed just above the previous tibiotalar joint. This was followed by three headless compression screws across the syndesmosis and lateral gutter. Patient underwent standard closure and was splinted. Anticipate patient being nonweightbearing for a minimum of four weeks given his age and bone quality with advancement to partial weight-bearing in cast and eventual weight-bearing hopefully by 8-12 weeks and have continued with bone health optimization with addition of bone stimulator to throw the entire kitchen sink at it.
