    Telehealth has come a long way. During the pandemic in 2020, SNFs faced immense pressure to minimize in-person visits and maintain high levels of patient care, which led to a surge in the use of telehealth and RPM. Since then, RPM has continued to play an important role in SNFs, particularly for managing patients with chronic conditions, improving care coordination, and reducing hospital readmissions. Nowadays, telehealth can provide a critical safety net for SNF patients, ensuring they receive continuous, high-quality care without overwhelming in-house medical teams. Here’s just one of many examples: Mrs. Aberman is an 82-year-old resident at a Skilled Nursing Facility (SNF) who has been recovering from surgery after a hip replacement. She had underlying health conditions, including high blood pressure and a history of heart disease, putting her at higher risk for complications. Mrs. Aberman’s SNF was equipped with a telehealth remote patient monitoring (RPM) system, where her vitals—heart rate, blood pressure, and oxygen levels—were continuously monitored by offsite telehealth staff. One evening, during a routine check, her heart rate spiked, and her oxygen levels dropped below a safe threshold. The remote telehealth team immediately flagged the abnormal readings and escalated the issue to the on-site nurses. As a result, the medical team was able to act quickly, administering oxygen and adjusting her medications, preventing a potentially life-threatening situation. Without the RPM system and the vigilance of the remote telehealth staff, her condition might not have been detected until much later, leading to a more severe health situation, possibly requiring hospitalization. The combination of technology and remote staff monitoring not only improved response times but also probably saved Mrs. Aberman's life!
