The company was founded in 2012 with the desire to help home health agencies identify patients on census at-risk for unplanned rehospitalization. This can have unintended impacts on the kind of care the patient receives while also creating challenges for hospice agencies.     We are honored that one of our own, Stephanie Skaggs, Hospice product lead, was selected as a member of the HSPN 2027 Future Leaders 2024 class. Stephanie is transforming how care is delivered in the hospice industry through the creation and management of Muse, a hospice care platform that is used as a tool to optimize care at the end of life. Stephanie is responsible for driving the product experience that over 2,000 clinical managers in hospice use on a daily basis in the United States. She is determined to deliver the best user experience and product features/functionality in order to ultimately drive and transform patient care, so that all hospice patients have support and care when they need it most, at the end of life.     ❓ How can home health and hospice agencies be sure their patients are in the right care setting for them and their families? 🔎 Take a look at our latest article to find out! https://lnkd.in/gqupD2Jr Coordination of patient transitions and understanding which patients are best suited to hospice care can be challenging. Too many patients who would benefit from hospice move too late to fully benefit, or don't transition to hospice at all. Instead, they spend more time in home health than is advisable given their condition, resulting in insufficient care. Patients whose trajectory shows that they are approaching end-of-life care should be identified early to facilitate conversations with family and patients about hospice resources, compassionate care, and end-of-life planning.
