He is very active, participating in rock climbing and weightlifting. His injuries include multiple rib fractures and a right scapula/glenoid fracture. He reports severe pain in the posterior right shoulder, but neurovascularly, the upper extremity is intact (NVI). PMH: No relevant past medical history. PE: Examination of the right upper extremity reveals ecchymosis and tenderness over the posterior shoulder. The patient experiences severe pain with any attempted range of motion. Sensation is intact in the axillary, median, radial, and ulnar nerve distributions. Radial pulse is 2+ and equal to the contralateral side. How would you manage this patient? Can you answer our free question of the day? A 27-year-old woman presents to her primary care physician with a 2-week history of increasing weakness. She initially felt weak during her evening workouts; however, it has now progressed to the point that at the end of the day, she cannot even go up the stairs. She feels fine in the morning and never has a problem during the day at work. She has no significant medical history and works as a programmer. Her temperature is 98.0°F (36.7°C), blood pressure is 124/84 mmHg, pulse is 82/min, respirations are 12/min, and oxygen saturation is 98% on room air. Physical exam reveals the finding shown in Figure A. An ice pack placed over the patient's eye causes resolution of the patient's symptoms. Which of the following would be the most appropriate in further evaluation of this patient? 1. Angiography of the brain 2. CT chest 3. MRI brain 4. PET scan 5.
