Amniotic membrane for recurrent macular hole. This is a highly myopic patient with a recurrent myopic macular hole that did not close after vitrectomy, ILM peel and gas. I gave her the option to try an amniotic membrane with silicone oil, and she elected to proceed after discussing the risks, benefits and alternatives. These Optos true color RGB and red-green photos show that the amniotic membrane was in place on post-op day 1. Autofluorescence shows that there is still an underlying hole, but it is well-covered by the amniotic membrane. The Topcon Healthcare Triton Swept-Source OCT also confirms placement of the amniotic membrane on top of the macular hole. I used a 8mm disc amniotic membrane from Opticyte (Merakris Therapeutics, Inc.). The sterile packaging allowed us to easily transfer the tissue to the operating room back table. The amniotic membrane was carefully handled and cut into quarters. One quarter was able to be folded and placed through the 25 gauge trochar with an ILM forcep. Fluid air exchange was performed and the amniotic membrane stayed in place. The Opticyte membranes are particularly good because no chemical reagents are used and less irradiation is given which keep the vital components of the membrane active. GPO4EYES has special pricing on them, which our practice definitely appreciates for uses like this.
