Our mission is to provide adults and their caregivers the tools and services they need to effectively navigate their health and life options. Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County. We work to coordinate services and support systems across a network of state and local organizations and help empower our community by providing access to information and support resources.     Join Sourcewise in recognizing October 9, 20241 as Ageism Awareness Day. Organized by American Society on Aging, Ageism Awareness Day is an opportunity to #TalkAboutAgeism. So what is ageism, anyway? Ageism refers to stereotypes (how we think), prejudice (how we feel) and discrimination (how we act) toward others or oneself based on age. Ageism can manifest in many different ways, some of which include an underrepresentation of older adults in the media, discrimination in healthcare settings, and lack of age equity in the workplace. As the Area Agency on Aging in Santa Clara County, Sourcewise is committed to empowering older adults in our community. One way we do this is by partnering with our network of grantees to fund services critical to bettering the lives of seniors.     Sourcewise is proud to have attended the Veterans Stand Down 2024 hosted by the Veterans Services Office (VSO) here in Santa Clara County. At this year's 3-day Stand Down, there was a claims clinic, opportunity to meet with the Santa Clara County Public Defender's Office, dental services, and a whole host of other services and support agencies available to veterans and their families. We were excited to share information about our programs and services to the hundreds of veterans who attended the event, and we look forward to continued collaborations with the VSO in the near future!

    The Senior Farmers' Market Nutrition Program (SFMNP) is officially closed for 2024. We closed out the 2024 season at the Alum Rock Village Farmers' Market. Thank you to everyone who showed up! Sourcewise is proud to be a local distributor of SFMNP vouchers and remain committed to empowering all older adults in Santa Clara County to live healthier lives.     A family caregiver is defined as someone who provides unpaid care for a family member or another individual (i.e friend or neighbor). Here in Santa Clara County, where the cost of living is among the highest in the country, the additional time needed to take care of a loved one can feel overwhelming and stressful. In a recent San Jose Spotlight article, Caregivers Network Supervisor Adrianna Stankovich covered the complex issues specifically facing Bay Area family caregivers and potential resources available.     Join Sourcewise in belatedly celebrating Grandparents Day from this past Sunday. September 8. Each year. more than 53 million Americans provide support to increase the health, quality of life, and independence of an older adult or adult with a disability. Another 2.1 million grandparents welcome younger relatives into their homes who cannot remain living with their parents. As part of our commitment to serving the older adults, adults with disabilities, and family caregivers in Santa Clara County, we want to take the opportunity to recognize all of the invisible but vital labor and care that family members provide to their loved ones. We see you and appreciate you! ❤️

    This past Sunday, August 25, Sourcewise was thrilled to join the Silicon Valley Pride Festival for the third year in a row! Team members from Information & Awareness and Caregivers Network represented Sourcewise speaking with over 300 members of the community. We loved the opportunity to share about our programs and services, while also celebrating some of the most vibrant members of our local community 🏳️🌈

    Thank you to the San Jose Vista Center for the Blind and Visually Impaired for inviting Sourcewise this past Wednesday, August 21. The Sourcewise team member gave a presentation that touched on all of the newest programs and services available to residents of Santa Clara County. The 50+ attendees were super engaged and asked a lot of great questions! As part of our commitment to serving older adults, adults with disabilities, and caregivers in Santa Clara County, we look for every opportunity to share about our resources directly with members of the community. Fun fact - the last time Sourcewise stopped by the Vista Center was over four years ago.

    Sourcewise now offers Linkages, our newest care management service for older adults and adults 18+ who are functionally impaired. Eligible individuals are assessed by Social Work Care Managers who develop a tailored care plan to ensure the individual is receiving the care they need to remain safely living at home. Eligible individuals may include individuals who experience: Impairment with completing one or more Activities of Daily Living (ADL); or Impairment with completing two or more instrumental Activities of Daily Living (IADL); or Unable to manage his/her own affairs due to emotional and/or cognitive impairment; or Be impaired by virtue of a significant event or circumstance that has occurred within the past 12 months To learn more, visit our website or give us a call (408) 350-3200 option 1 to learn more today! https://lnkd.in/g9Xtqp7C.
