Our mission is to provide adults and their caregivers the tools and services they need to effectively navigate their health and life options. Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County. We work to coordinate services and support systems across a network of state and local organizations and help empower our community by providing access to information and support resources.     This past Sunday, August 25, Sourcewise was thrilled to join the Silicon Valley Pride Festival for the third year in a row! Team members from Information & Awareness and Caregivers Network represented Sourcewise speaking with over 300 members of the community. We loved the opportunity to share about our programs and services, while also celebrating some of the most vibrant members of our local community 🏳️🌈

    Thank you to the San Jose Vista Center for the Blind and Visually Impaired for inviting Sourcewise this past Wednesday, August 21. The Sourcewise team member gave a presentation that touched on all of the newest programs and services available to residents of Santa Clara County. The 50+ attendees were super engaged and asked a lot of great questions! As part of our commitment to serving older adults, adults with disabilities, and caregivers in Santa Clara County, we look for every opportunity to share about our resources directly with members of the community. Fun fact - the last time Sourcewise stopped by the Vista Center was over four years ago.

    Sourcewise now offers Linkages, our newest care management service for older adults and adults 18+ who are functionally impaired. Eligible individuals are assessed by Social Work Care Managers who develop a tailored care plan to ensure the individual is receiving the care they need to remain safely living at home. Eligible individuals may include individuals who experience: Impairment with completing one or more Activities of Daily Living (ADL); or Impairment with completing two or more instrumental Activities of Daily Living (IADL); or Unable to manage his/her own affairs due to emotional and/or cognitive impairment; or Be impaired by virtue of a significant event or circumstance that has occurred within the past 12 months To learn more, visit our website or give us a call (408) 350-3200 option 1 to learn more today! https://lnkd.in/g9Xtqp7C.

    Final call for volunteers to become state-registered #Medicare insurance counselors with Health Insurance Counseling & Advocacy Program (#HICAP) by Sourcewise. No-cost, extensive training is available to interested applicants. Applications close July 26, 2024. HICAP is part of the national network of State Health Insurance Assistance Program (SHIP) and is the Santa Clara County source for unbiased and objective Medicare-related #counseling and information. We need counselors to help us continue assisting Medicare beneficiaries, their families, and caregivers understand their Medicare benefits and healthcare options. Retired professionals and bilingual speakers are encouraged to apply. Spanish speakers are especially needed.     On June 25, 2024, Sourcewise partnered with the Archstone Foundation, California Department of Health Care Services, California Department of Aging, and Family Caregiver Alliance / National Center on Caregiving to present an online webinar: Understanding the CalAIM Information & Referral Highway to ECM Enrollment: What Organizations Needs to Know. The CalAIM program offers important opportunities to address social and environmental drivers of health through Enhanced Care Management (ECM) and Community Supports (CS). Senior Director of Operations, Elizabeth Rodriguez, participated in the webinar discussion and was able to share some day-to-day insights of what it looks like to run ECM and CS programs.     Participants in our Digital Connections program recently gathered at the Santa Clara Senior Center to receive personalized training on how to use their new iPads. At this training, they learned how to set up their accounts, how to download apps, and how to safely browse the internet. Our digital learning trainings vary according to the needs, interests, and skill levels of each client. We are happy to provide individualized resources and solutions to every client.     This summer, Sourcewise is coordinating Intergenerational Activities at The Lodge, a senior housing complex in Morgan Hill. Through friendly board games and fun arts and crafts projects, this new initiative brings volunteers and seniors together to increase socialization and connection. Past activities include Jenga, Uno, and painting! Do you have a teen or a friend looking to volunteer this summer? Consider spending some time with our older adults. Individuals of all ages, languages and cultural backgrounds are needed for our ongoing volunteer opportunities such as digital literacy, beautification projects, and more around Santa Clara County.     Sourcewise is looking for a full-time Social Work Care Manager to join the Multipurpose Senior Services Program (MSSP). As part of our commitment to helping all adults in Santa Clara County navigate their health and life options, we offer several different care management programs depending on the needs of each client. MSSP works with frail older adults 65 years of age and older who are at risk of nursing home placement and would like to remain living at home. Our care managers work with the client, their loved ones, and medical professionals to develop and implement an individualized care plan.
