NAACOS recently submitted feedback to the Physician-Focused Payment Model Technical Advisory Committee (PTAC) on how to address the needs of patient with complex chronic conditions or serious illnesses in population-based total cost of care models. In comments, NAACOS recommended that policymakers design specific program policies to account for high-cost, high needs beneficiaries who are significantly different from the average traditional #Medicare beneficiary, including financial methodologies, attribution models, and quality measurement. As we approach the Centers for Medicare & Medicaid Services (CMS) 2030 goal to have all traditional Medicare beneficiaries in #accountablecare relationships, it is critical to uncover and address the current barriers to #valuebasedcare adoption. In this Health Affairs Forefront article, authors Clive Fields and Gary Jacobs outline these challenges and offer four pillars that policy makers can incorporate into value models to support the transition to VBC: 📈Full risk sharing ensures that accountability for a patient’s care remains with the health care provider. 💰Upfront capitated payment to ACOs, or pre-payment, creates stability for health care providers, allowing them to focus on patient care. 📶Providers need financial predictability to transition into VBC payment models. 🔄VBC models must ensure sustainability for ACOs to promote value-based care over the long term, and providers should not be penalized for their financial success in the model, a phenomenon known as the ratchet effect. Today, NAACOS and the Health Care Transformation Task Force (HCTTF) released a new resource on strengthening #patientengagement in ACO governance, care delivery redesign, and individual care planning. The resource outlines recommendations for the Centers for Medicare & Medicaid Services (CMS) that were informed by ACOs and patient and consumer advocacy organizations, with short- and long-term recommendations including: 1️⃣ Remove the current one-size-fits-all approach to ACOs’ communications with beneficiaries. 2️⃣ Work to expand and align waivers that provide direct benefits to beneficiaries. 3️⃣ Ensure ACOs have adequate guidance to solicit beneficiary input and feedback, establish community partnerships, and incorporate these perspectives into their work. The proposed 2025 Medicare Physician Fee Schedule rule was released yesterday by the Centers for Medicare & Medicaid Services (CMS) and includes a number of changes to the #Medicare Shared Savings Program (MSSP). NAACOS appreciates CMS's continued efforts to address the significant anomalous and highly suspect billing activity identified by ACOs in late 2023. Long-term solutions are critical for ensuring stability in the program. Despite a number of positive changes, CMS has yet to address significant issues with MSSP's financial benchmarking policies and quality reporting requirements. NAACOS will continue to advocate for sensible changes that increase participation and create long-term sustainability for the program.
