This week is National Community Health Worker (CHW) Awareness week. CHWs are invaluable in our health care system, tackling health inequities and disparities head-on. Hundreds of studies have been conducted over the past decade on CHW efficacy, finding that CHWs improve chronic disease management and care amongst vulnerable populations, increase post-hospital outcomes like getting primary care, mental health improvements, reducing readmissions, and much more.
