Through a partnership between Nexus Children’s Hospital – Dallas and Pediatric Acute Care Associates of North Texas, Nexus is giving pediatric patients at Medical City Hospital Dallas access to coordinated step-down care. Led by Dr. Michele Papo, this partnership creates a more continuous experience for those moving from a pediatric intensive care unit (PICU) to Nexus’ transitional care unit. “We are thrilled to partner with Dr. Papo and Pediatric Acute Care Associates of North Texas to provide a more seamless transition from intensive to transitional care,” said Nexus Children’s Hospital – Dallas CEO Roger Caron. Contractures — the permanent tightening of muscles and other tissues that causes joints to shorten and become stiff — can easily develop in the early stages of brain injury recovery. As patients emerge into consciousness, sympathetic brain activity can cause contractures and other secondary injuries. In the Rise Program at Nexus, our goal is to maximize patients’ future potential through medicine and therapies that combat inflammation and sympathetic overactivity. While spending time with friends, Alex Gaytan was robbed at gunpoint, sustaining a gunshot wound to the head. He knocked on four nearby doors until someone answered, administered first aid and called for an ambulance. In the weeks that followed, Alex underwent multiple surgeries to address intracranial and facial injuries, including a broken mandible. Tracheostomy and gastrostomy tubes were placed, as well as an external fixator to support his facial bones as they healed. By the time he left Nexus, Alex’s tracheostomy and gastrostomy tubes had been removed and he had regained roughly 70% of his physical abilities, according to his aunt. How can we implement collaborative care models in Houston and beyond? At the 2024 Changing Care Together Summit, Amy Boutwell, MD, MPP, will discuss how organizations across the country have improved care coordination while lowering costs. Founder of Collaborative Healthcare Strategies, Dr. Boutwell is a nationally recognized leader of efforts to reduce readmissions and improve care for the highest risk patients. She has advised large-scale initiatives to support healthcare delivery system transformations at the local, state, and national levels. Dr. Boutwell’s methods are distinctively systems-based, data-informed, and root-cause focused. Haven’t registered for the summit yet? There’s still time to join the conversation!
