Scenario: During your SOC assessment, you note that Mrs. Elliott is being referred to you for treatment and management of a Kennedy Ulcer on her sacrum. You will provide treatment while the patient is awaiting referral to in-home hospice care. You observe the area on her sacrum to be black and swollen with irregular edges. Mrs. Elliott has no other ulcerations identified during her first complete skin assessment. How should M1306 – Does the patient have at least one Unhealed Pressure Ulcer/Injury at Stage 2 or Higher and M1324 – Stage of Most Problematic Unhealed Pressure Ulcer/Injury that is Stageable be answered? A: M1306 = 0, No; M1324 = NA, Patient has no pressure ulcers/injuries or no stageable pressure ulcers/injuries B: M1306 = 0, No; M1324 = Dash (-) C: M1306 = 1, Yes; M1324 = 3, Stage 3 D: M1306 = 1, Yes; M1324 = 4, Stage 4 Correct answer: A, M1306 = 0 – No; M1324 = NA – Patient has no pressure ulcers/injuries or no stageable pressure ulcers/injuries Rationale: Per Question 2 of the July 2024 Quarterly Q&As, CMS clarified that end-of-life ulcers, also known as Kennedy ulcers or terminal ulcers, result from organ failure and are not considered pressure ulcers. Therefore, these ulcers are not reported on the OASIS pressure ulcer items. Reference: CMS Quarterly Q&As, July 2024
