    New post - some thoughts on where we are today in value-based care it gets a ton of airtime as the way to "fix" US healthcare, but maybe we should give more thoughts to other ways to improve things I talk about a few other ideas in the post (link in comments)

    On the final day of the healthcare 101 course (starts in a couple weeks!) we talk about some of the practical and ideological challenges with implementing value-based care Value-based care is one of those things that sounds awesome in theory, but when you get to the practical parts of it because very challenging - What is the baseline you're comparing to? is it the same for everyone participating in the program, or does each hospital get their own? If they show they can perform well, do you lower the baseline the following year? - What are the right quality metrics? A lot of times you're looking for short-term quality metrics you assume will yield better long term outcomes. But which ones are right? How easy are they to game? How hard is it for a hospital to get them? - When should you pay providers for improving outcomes? If you pay it all in the beginning, it makes it easier to invest in software/staff to participate in the program but you'll likely pay less accurately since you're guessing the amount. If you pay it all at the end, it might be more accurate but then only hospitals that can wait a long time for cash will participate + makes it harder to invest in the things you need upfront. There's also a bunch of ideological questions that aren't so straight forward. We go through all of this and have a discussion in the healthcare 101 course, enrollment ends in a couple weeks. I'm now a B2Boi The Out-Of-Pocket courses have been designed to get people up to speed quickly on different parts of healthcare (how to do data analyses, set up call centers, bring a healthcare product to market, and more) We've been running the Out-Of-Pocket courses for 2+ years now, and we've gotten requests for bulk discounting, recorded versions of the course, custom modules, etc. We've been doing this ad-hoc for large consulting firms, agencies, tech companies, etc. But now we have a ~*process*~ for that.
